Job Summary

The Transition Coordinator is responsible for planning, coordinating, and supporting transition services for students with disabilities across the continuumfrom early intervention (BabyNet/Part C) through school-age services and into post-secondary outcomes. This position ensures compliance with federal and state regulations while promoting smooth, student- and family-centered transitions at all stages, including entry into preschool special education and exit to adulthood.


Key Responsibilities

Early Childhood Transition (Part C to Part B)

  • Coordinate transition activities for children exiting early intervention (BabyNet) into preschool special education or other community-based programs.
  • Support families in understanding differences between early intervention (IFSP) and preschool special education services (IEP).

School-Age Transition (Part B to Post-Secondary)

  • Coordinate transition planning for students with disabilities (typically ages 1421) as part of the IEP process.
  • Support development of measurable postsecondary goals and transition services aligned to student strengths, preferences, and interests.
  • Participate in student-centered IEP meetings with a focus on transition outcomes.
  • Assist students in accessing work-based learning experiences, internships, job shadowing, and vocational training opportunities.
  • Collaborate with families and staff to prepare students for post-secondary education, employment, and independent living.

Collaboration & Partnerships

  • Develop and maintain partnerships with community agencies, including early intervention providers, vocational rehabilitation, colleges, employers, and other service organizations.
  • Connect families with adult service agencies and community resources.
  • Collaborate with general and special education staff, related service providers, and administrators across grade levels.

Compliance & Data Management

  • Ensure compliance with IDEA (Part C and Part B) and all state transition requirements.
  • Monitor and analyze transition data, including early childhood timelines and post-school outcomes, to inform program improvement.

Training & Support

  • Support district initiatives related to early childhood services and college/career readiness.
  • Assist families by providing guidance and resources throughout all transition stages.

Qualifications

  • Any combination of education and experience providing the required skills and knowledge for successful performance qualifies.  Typical qualifications include:
    • High school diploma or equivalent
    • Bachelor's degree in Special Education, Early Childhood Education, Counseling, Social Work, or a related field preferred.
  • Experience working with children and students with disabilities across early childhood and/or school-age settings.
  • Knowledge of IDEA Part C and Part B regulations, including transition requirements.
  • Experience collaborating with families and community agencies.
