Care Dimensions Healthcare is currently seeking a compassionate, community-centered, and motivated Full-Time Community Health Worker (CHW) to join our growing team. This position will report to our Corona office and primarily support patients throughout the Hemet area.

This is a full-time Monday through Friday position with an hourly pay range of $25-$30 per hour, depending on experience.

Position Overview

Under the supervision of the Program Lead, the Community Health Worker plays an important role as part of our multidisciplinary care team. The CHW works closely with patients, families, providers, and community partners to support whole-person health and improve access to care and resources.

This role includes a combination of outreach, engagement, care coordination, patient support, and field-based community work as part of our Enhanced Care Management (ECM) services.

Key Responsibilities

  • Support patient care through outreach, follow-up, assessments, scheduling coordination, and quality metric initiatives
  • Engage patients and families in navigating healthcare systems and overcoming barriers to medical, behavioral health, and social services
  • Collaborate closely with the care team to support a holistic, patient-centered approach to care
  • Conduct field-based outreach and maintain ongoing communication with patients and community partners
  • Connect patients to community resources, social services, health education materials, and supportive programs
  • Provide advocacy, case management support, and compassionate guidance throughout the care journey
  • Assist with documentation and coordination activities as needed
  • Support additional program and patient care activities assigned by leadership

Qualifications

Preferred qualifications include:

  • Community Health Worker (CHW) Certification
  • OR a minimum of 2 years of experience as a CHW, Promotora, Health Navigator, or in community outreach, customer service, medical, behavioral health, or substance use disorder settings
  • Current and valid California Driver's License
  • Comfortable traveling throughout Riverside County to support patients and community events
  • Strong communication, relationship-building, and organizational skills
  • Ability to work independently while also collaborating effectively within a team environment
  • Experience using electronic medical records and computer systems
  • Spanish-speaking candidates are encouraged to apply
