Job description

Physical Therapist (PT) - Skilled Nursing Facility - Lewistown, MO
Seeking a travel Physical Therapist to provide skilled rehabilitation services in a long-term care setting. The clinician will evaluate, treat, and document patient progress using evidence-based practice and standardized assessments. Responsibilities include developing and adjusting individualized plans of care, performing manual techniques, therapeutic exercise, ambulation training, wound care, and use of assistive devices. The therapist will collaborate with interdisciplinary team members, participate in care conferences, and complete regulatory documentation and treatment minute tracking.
Minimum Qualifications
  • Entry-level to experienced PTs accepted. New graduates are acceptable if they have Level II clinical experience (not solely Level I observation).
  • Minimum 1 year of PT clinical experience required.
  • Experience in long-term care or rehab required.
  • Active Missouri state license required. Compact license accepted.
  • Ability to perform wound care, therapeutic exercise, ambulation training, manual techniques, and use of assistive devices.
  • Proficiency with electronic documentation systems and accurate treatment minute recording.
  • Ability to supervise PT assistants, rehab aides, and students when applicable.
  • Adherence to APTA and state practice acts, professional code of conduct, and ethics.
  • BLS certification may be required from AHA or ARC.

Preferred Qualifications
  • Prior experience in skilled nursing facility or long-term care preferred.

Assignment Details
  • M-F, 8 hour days. Times are flexible as long as consistent and communicated with Director of Rehab.
  • Start time requested before 6:00 AM and no later than 10:00 AM.
  • 35 GWW.
  • No weekends required unless a holiday falls within M-F; then hours may be made up on the weekend.
