Job description

Registered Nurse

Work Schedule: 2nd shift 1:45pm-10pm or 3rd shift 9:45pm-6am

Academic Req: Required – Registered nurse license; Preferred – Bachelor's of Science in Nursing

Certifications: Good standing as a licensed Registered Nurse with the State of Texas Board of Nursing; current BLS certifications (PALS preferred), First aid, CPR, Emergency behavior intervention

Work experience: Required – At least one year of experience as a RN; Preferred – pediatric nursing experience

Critical Action Items & Measurable Deliverables:

1. Meet and maintain all federal and state regulatory guidelines and standards applicable to this position.

2. Provide triage services to assess patient needs and evaluate symptoms. This includes answering patient inquiries, providing patient teaching, and determining the urgency and appropriate course of action.

3. Have extensive knowledge of medical documentation to be able to generate, decipher, and correct for proper documentation methods in accordance with state and federal guidelines.

4. Perform nursing duties and tasks relevant to this position in compliance with all regulatory authorities and program protocols including, but not limited to, active and ongoing nursing assessments, charting, scheduling appointments, routine observations, isolation observations, wellness checks, immunizations, medication administration, and time internal reports, when needed.

5. Ensure patient safety by assessing and monitoring health conditions, assist with procedures and operate medical equipment, administer vaccines, medications and treatments ordered by healthcare provider, monitor for side effects and reactions, and provide teaching to patients and staff.

6. Arrange and coordinate appropriate health services, including urgent, emergent, and preventative health services for children in care.

7. Conduct health assessments based on established protocols and accurately document medical findings, including validating systems of record for required facility documentation and reporting to meet agency, federal, and state guidelines within required timeframes.

8. Supervise, direct, and delegate when appropriate to execute medical services reflected in healthcare plans and treatment orders, including medication administration, to ensure that all treatment orders directed by the prescribing healthcare professional are carried out as ordered and monitor the patient for improvement.

9. Gather, collect, and clarify patients' current condition and past medical history.

10. Communicate with pharmacies on ordering prescription medications, check medication deliveries for accuracy and compliance with orders and remediate any discrepancies.

11. Maintain adequate over the counter medication supplies for each service population facility and arrange proper disposal of unused and expired medications.

12. Maintain documentation of controlled drugs that may be available to include up to the date reconciliation of all scheduled drugs used in the facility through the facility's protocols and procedures.

13. Administer medication as ordered by prescribing healthcare provider and complete accurate and timely documentation in electronic medical records and Medication Administration Record.

14. Effectively communicate with Certified Nurse Practitioner and Lead Medical Coordinator as needed for reporting findings concerning for patient's health and well-being.

15. Communicate with Lead Medical Coordinator and other medical staff to devise individual plans of care and acts as a nurse support to other medical staff.

16. Implement Compass Connections safety protocols including evacuating with children and other staff in case of an emergency.

17. Maintain confidentiality of health care records and information in keeping with HIPAA and Compass Connections confidentiality and Personally Identifiable Information (PII) protocol and procedures.

18. Address other job duties as assigned or for unforeseen matters that require attention to maintain a fluid and fully functional program.

Other Responsibilities:

1. Plan, organize and, prioritize daily work using sound judgement.

2. Monitor established health care plans for service population at the sites within the timeframe established by our State and Federal partners.

3. Know and recognize the signs of abuse, neglect, and exploitation and sexually provocative behavior and implement professional response, reporting and documenting procedures.

4. Ensure accurate documentation in accordance with agency policies and procedures, regulatory requirements and standards, and program timeline requirements.

5. Meet all program and federal partners' documentation and processes requirements.

6. Other duties as assigned.

Requirements:

1. Pass a pre-employment drug screen and random drug screens throughout employment.

2. Provide proof of work eligibility status upon request.

3. Pass a pre-employment and biennial criminal background checks.

4. Demonstrate the ability to:

a. Work collaboratively with other staff members, service providers and professionals.

b. Communicate effectively in writing and verbally in English.

c. Work in a fast-paced environment and make decisions based on the needs of the program and service population, while maintaining emotional control and professional composure at all times.

d. Maintain computer literacy required to meet the responsibilities of the position.

e. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team.

5. Demonstrate a working knowledge of all Compass Connections policies and procedures.

6. Must provide current physical-medical release and provide proof of immunizations, titers, or declinations for the following:

a. MMR

b. Varicella

c. Hep B

d. Hep A

e. Tetanus

f. PPD

g. Meningococcal (preferred)

7. Must be able to perform physical activity such as extensive walking, stretching, and bending.

8. Must be able to work in a team environment.

9. Must be able to maintain professional people skills, including oral, written, listening, and non-verbal communication skills.
