Registered Nurse (RN) – Home Health
Help patients recover where they’re most comfortable—at home. We are seeking an experienced Home Health Registered Nurse (RN) who is passionate about providing exceptional patient care while working independently in the field. This role is ideal for an RN with strong assessment skills, Home Health experience, and proficiency with OASIS and HomeCare HomeBase.
Schedule
- Monday – Friday
- 8:00 AM – 5:00 PM
- Some weekend availability preferred
- Approximately 1 weekend per month
- 1 on-call night per week
Territory
Coverage includes:
- Escambia County
- Santa Rosa County
- Okaloosa County
- Walton County
Primary service area will be Okaloosa and Walton Counties.
Mileage Reimbursement: $0.43 per mile
Required Qualifications
- Active Florida Registered Nurse (RN) License (Compact License accepted)
- HomeCare HomeBase (HCHB) experience required
- OASIS documentation experience required
- Associate's or Bachelor's Degree in Nursing
- Valid driver's license, reliable transportation, and current auto insurance
- Current CPR certification (if required)

Responsibilities
- Perform comprehensive assessments for high-risk home health patients and develop individualized care plans.
- Deliver skilled nursing care while promoting patient independence, health maintenance, and disease prevention.
- Collaborate with physicians, therapists, social workers, discharge planners, and other healthcare professionals to coordinate patient care.
- Identify barriers to care and arrange non-medical resources such as transportation or community support services to improve patient compliance.
- Monitor patient progress, reassess care plans, and communicate changes in condition to physicians and the interdisciplinary care team.
- Complete accurate and timely clinical documentation, including OASIS assessments and charting in HomeCare HomeBase (HCHB).
- Perform skilled nursing procedures including wound care, PICC/IV care, lab collection, drain management, ostomy care, and post-operative assessments.
- Promote preventative care, patient education, and disease management to reduce hospitalizations and improve outcomes.
- Advocate for patients by ensuring privacy, confidentiality, and access to appropriate healthcare resources.
- Maintain detailed clinical, functional, and quality documentation in compliance with regulatory and company standards.
