RN – Case Manager

Navajo Area Indian Health Services administers health centers, and hospitals, providing health care to approximately 201,516 members of the Navajo Nation. The Navajo Nation is the largest Native tribe in the United States and has the largest reservation, which encompasses more than 25,516 square miles in northern Arizona, western New Mexico, and southern Utah, with three satellite communities in central New Mexico. The NAIHS is the primary provider of inpatient, ambulatory care, preventive and community health, and environmental health services for members of the Navajo Nation and the San Juan Southern Paiute Tribe. Requirements (Duties include but are not limited to)
  • Providing professional nursing care principles, practices and procedures required to assess needs of patients;
  • Knowledge of pharmaceuticals, their desired effects, side effects, and complications of their use; recognizing adverse signs and symptoms which are not always apparent and to react promptly in emergency situations including the implementation of life saving procedures;
  • Providing counseling, guidance and health instruction to patients; and
  • Identifying disease entities in order to recognize complications and therapeutic interventions required.
  • Conduct comprehensive assessment of patients' physical, mental, and psychosocial needs
  • Develop care plans to prevent disease exacerbation, improve outcomes, increase patient engagement in self-care, decrease risk status, and minimize hospital and ED utilization
  • Utilize behavioral strategies help patients adopt healthy behaviors and improve self-care in chronic disease management. Promote self-management goals.
  • Assist patients in navigating the health care system. Coordinate Specialty care, follow-up on test results and other care coordination needs.
  • Follow-up with patients within 24 hours on inpatient discharge & within 48 hours of ED visit notification
  • Partner with external case management programs to coordinate care
  • Ongoing evaluation and documentation of patient progress/ risk status Document in EMR; communicate with care teams
Qualifications
  • Candidates must possess a bachelor's degree in Nursing and eligibility under the Superior Academic Achievement provision -OR- Possess of a bachelor's degree in Nursing and at least one full year of professional nursing experience-OR- Possess a diploma or associate degree in professional nursing
  • Must possess a current, valid, unrestricted nursing license in a state, the District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States.
  • Must have a minimum of 12 months experience as a Case Manager during the last 36 months; Case Management Certification is a plus
  • Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) as approved by the American Heart Association (AHA) is mandatory for all specialties.
  • Evidence of current immunizations is required
