Total Care 2U is a localized medical practice run by doctors who are dedicated to providing comprehensive primary care to chronic and transitional patients in the comfort of their home. Additionally, we coordinate services with home health agencies, hospitalists, assisted living, independent living, and CCRCs. Our focus is partnering with doctors and facilities to create a patient-centered network for seniors with chronic illnesses. Each Total Care Team comprises of an experienced primary care physician and a specialized nurse practitioner. Total Care 2U provides quality service to the aging population, and creates better clinical outcomes to patients. We manage the unique needs of every patient and make sure that each visit is comfortable in the process.
