THE OPPORTUNITY

Because traditional primary care doesn’t always provide optimal care to individuals diagnosed with multiple chronic conditions, we are embarking on a new Outpatient Chronic Care Clinic. With an intensification of up-front care for the chronically ill, appropriate interventions can reduce the frequency of emergent health crises and avoid ongoing hospitalizations.

This clinic offers quality chronic care management, coordinated with our preferred specialist network, in an outpatient setting. Our ideal candidate possesses the ability to manage patients with multiple comorbidities to curb the possibility of inpatient admission. As a continuum of care provider, this clinic will work with existing primary care providers to identify patients most at risk of hospital admission. The clinic creates an environment for regular monitoring and interventions for conditions.

Our clinic recognizes that people with chronic illnesses, such as diabetes, arthritis and cardiac problems, oftentimes are pulled between multiple specialists to monitor and effectively treat their multiple conditions. In our model, you - as our Top Gun provider - in coordination with our preferred specialist network are the provider that can effectively manage all conditions and prepare the best health plan strategy for each unique patient situation.

Diabetes, cardiac disease, and other chronic conditions account for 80 percent of health-care spending in the United States. When these conditions are poorly managed, patients are likely to visit the emergency room or the hospital, often at great cost. Our mission is to treat our patients with quality care in the comfort of an outpatient setting, reducing the likelihood of inpatient admission or ER visit.

**The clinic keeps normal 8am-5pm hours Monday through Friday.
