Cardiologist for the White River Indian Hospital in Whiteriver, AZ
Contract Coverage Dates: June 1, 2018 Through May 31,2019
Option to Extend:
Option Year 1: June 1, 2019 through May 31, 2020
Option Year 2: June 1, 2020 through May 31, 2021
Option Year 3: June 1, 2021 through May 31, 2022
Option Year 4: June 1, 2022 through May 31, 2023
Deadline to submit providers: May 24, 2018 5:00PM Mountain
Arizona State Licensure Required
Outpatient with phone consult call
Current Cardiology clinics are scheduled for Thursday afternoon and Friday Morning
Clinic Day: 8:00am -12:00pm and/or 12:00pm-4:00pm
Anticipate 24 half-day clinical sessions (12 full day) over the course of a year
