Physicians lead a multidisciplinary team and serve on the frontlines delivering preventive and urgent medical care in the homes of complex, frail, and vulnerable patients. They are looking for BE/BC Family Medicine, Internal Medicine, Emergency Medicine, or Geriatric Physicians. The schedule will be M-F 8-5. They are looking for Candidates who are experience managing complex, polychronic patients and comfort with high acuity.
