Signature Albany Home Health & Hospice is seeking a compassionate and dedicated Medical Social Worker (MSW) to join our interdisciplinary Home Heath and Hospice team. This full-time role is ideal for a social worker passionate about end-of-life care, psychosocial support, and patient advocacy.

As a Home Health and Hospice Medical Social Worker, you will provide critical emotional support, care coordination, and community resource guidance to patients and families navigating life's most meaningful transitions.

🔎 What You'll Do (Key Responsibilities)

• Conduct comprehensive psychosocial assessments for hospice patients and families
• Develop and implement care plans based on social and emotional needs
• Provide grief counseling, bereavement support, and crisis intervention
• Coordinate community resources, referrals, and support services
• Serve as a liaison between patients, families, and healthcare providers
• Collaborate with the interdisciplinary hospice team (RN, physician, aides)
• Educate caregivers on coping strategies and end-of-life planning
• Participate in quality assurance and performance improvement initiatives

What You Bring

• Bachelor's or Master's degree in Social Work (MSW or BSW) from an accredited program
• 1+ year of healthcare experience (hospice or home health preferred)
• Understanding of hospice philosophy, end-of-life care, and grief processes
• Strong communication, organization, and interpersonal skills
• Ability to work independently in a home health / field-based setting
• Current CPR certification
• Valid driver's license and reliable transportation
