Medical Case Management & Care Coordination Services (VTARNG)GD Resources LLC is seeking to hire qualified healthcare professionals to support Medical Case Management and Care Coordination Services for the Vermont Army National Guard (VTARNG). These positions play a critical role in supporting Soldier medical readiness, care coordination, and medical documentation management.
1. Lead RN Case ManagerGD Resources LLC is seeking a highly experienced Registered Nurse to serve as the Lead RN Case Manager and Contract Manager. This individual will oversee all case management operations and serve as the primary point of contact for Government coordination.
Responsibilities: - Lead and supervise all case management and care coordination activities
- Serve as primary liaison with military medical leadership and Government representatives
- Oversee RN Case Managers and Medical Records Technicians
- Manage complex Soldier medical readiness cases
- Ensure compliance with Army medical readiness regulations and HIPAA standards
- Conduct monthly case reviews and reporting
- Support MEB/PEB preparation and readiness operations
- Ensure accurate and timely documentation in eCase and MEDCHART systems
Requirements: - Active Registered Nurse (RN) license (Vermont or compact)
- Minimum 4 years nursing experience
- At least 2 years supervisory or team lead experience
- Experience in case management or clinical coordination
Preferred: - BSN or higher
- Certified Case Manager (CCM) certification
- Experience with military medical systems (MEDCHART, MEDPROS, MHS GENESIS)
2. Registered Nurse Case ManagerGD Resources LLC is seeking qualified Registered Nurses to perform medical case management and care coordination duties supporting Soldier readiness and healthcare coordination.
Responsibilities: - Manage assigned Soldier medical readiness cases
- Create and update eCase profiles
- Coordinate care between Soldiers, providers, and military units
- Document all medical encounters and updates in a timely manner
- Support SRP, PHA, and readiness events
- Assist with MEB/PEB case preparation and documentation
- Ensure compliance with HIPAA and medical regulations
- Track Soldier medical readiness status and follow-up care
Requirements: - Active Registered Nurse (RN) license (Vermont or compact)
- Minimum 2 years clinical nursing or case management experience
- Nursing diploma or degree from an accredited program
Preferred: - BSN degree
- Experience with MEDCHART, MEDPROS, or MHS GENESIS
- Military healthcare experience
3. Medical Records TechnicianGD Resources LLC is seeking detail-oriented Medical Records Technicians to support medical documentation, records processing, and data management for Soldier readiness cases.
Responsibilities: - Scan, upload, and manage medical records in electronic systems
- Maintain accurate Soldier medical files and documentation
- Support case managers with records tracking and updates
- Process medical documentation within required timelines
- Assist in preparation of records for medical boards (MEB/PEB)
- Maintain compliance with medical records standards and HIPAA requirements
- Support monthly reporting and data validation
Requirements: - High School Diploma or GED
- At least 1 year experience in medical records or healthcare administration
- Strong attention to detail and organizational skills
Preferred: - Experience with military medical systems (MEDCHART, MEDPROS, MHS GENESIS)
- Knowledge of medical terminology and documentation processes
