The Case Manager will support transition-aged youth (14-24 years old) by providing one-on-one counseling, including crisis intervention, advocacy, and referral services; develop relationships of trust with clients for the purpose of providing motivational counseling. This position will work with students enrolled in our introductory media education programs as well as on-site interns in our
workforce program.

The Case Manager will utilize a strength based approach to complete biopsychosocial assessments of newly enrolled participants, co-develop individualized action plans and update assessments on a quarterly basis. This position supports youth throughout their interaction with YR Media (YR).

  • Provides case management for 65-80 young adults engaged on site at YR’s Oakland headquarters.
  • Monitors and support youth participation in advancing through YR’s education and workforce programs with successful outcomes.
  • Administers biopsychosocial assessments, case management and counseling services.
  • Establishes an individualized action plan for each participant in need.
  • Facilitates weekly individual sessions with participants in need.
  • Collaborates with outside stakeholders (i.e., teachers and guidance counselors, parole officers, social workers, etc) as needed.
  • Maintains documentation requirements (individualized action plans, progress notes, incident reports, etc.
  • Supports youth in managing the social services sector (e.g., accessing public benefits, housing, etc.).
  • Serves as liaison/advocate between YR staff and youth participants.
  • Provides information and referrals to off-site service providers that can meet the support needs of youth beyond YR’s capacity and scope of work.
  • Provides crisis intervention, conflict resolution and counseling to individuals.
  • Promotes youth self-sufficiency and independence.
  • Complies with all mandated reporting procedures.
  • Maintains detailed, confidential records on all participants served, and collaborate with management on all data collection and reporting requirements (such as service hours completed for government grant requirements)
  • Attends meetings, trainings, workshops, as assigned, to build collaboration with outside stakeholders and for ongoing professional development.
  • Participates in YR staff meetings and lead or organize organization-wide trainings on best practices in supportive services for young adults.
  • Other related duties as assigned.
Work Environment:
Approximately 50% of time spent on the job involves use of a video display terminal; therefore, must be able to sit for extended periods and to utilize standard ergonomics practices. Primarily works in an office environment, and at times may require the following activity: lifting and carrying materials weighing up to 15 pounds, kneeling, bending. Must be able to use standard office equipment.

Minimum of 3 years direct service experience
Proven experience in and knowledge of case management methods, principles, techniques and resources (including information/referral, assessment, interviewing, client advocacy, crisis and risk intervention, resource utilization)
Ability to quickly adapt to new technology, automated systems and databases
Experienced in supporting system-engaged or previously system-engaged youth in achieving goals
Lifting and carrying materials weighing up to 15 pounds, kneeling, bending
Bachelor’s degree in social work, psychology, public health, counseling or equivalent education and experience
Knowledge in: Standard Office Equipment
