LivinRite Home Health is looking for a Full-time experienced Social Worker. Looking for a compassionate, experienced, and intuitive Social Worker who can provide the highest service and commitment to our patients. Someone that understands the complexities and sensitivity needed when caring for home health patients and their families and thrives in this setting. As a Social Worker, you are a vital part of our interdisciplinary team that works cohesively to provide the best care possible.
Our Social Worker is responsible for assessing our home health patients and their family to identify various psycho-social, financial, environmental, and community needs. You'll work to educate the patients and their family regarding the philosophy of home health, while playing a vital role in patient care planning. Your support and collaboration will connect our patients with a variety of community resources and partners.
Evaluates the social, emotional and environmental factors to determine the capacity of the patient and caregivers.
Participates in the completion of a psychosocial assessment delineating specific problems, goals, and interventions.
Coordinates with interdisciplinary team members to develop and implement a plan of care that includes potential problems and areas in social work interventions.
Provides initial assessment of home health patients.
Presents home health services to potential patients and to other healthcare professionals.
Masters or Bachelors of Social Work from an accredited graduate program.
Licensed Social Worker with the state of Virginia.
Minimum of two years post-graduate clinical experience.
Ability to work with patients and families dealing with life-threatening illness required.
Excellent listening and communication skills and effective interpersonal skills.
Previous hospice experience preferred.
Knowledge and/or experience in patient education. Prefer experience within long-term, home health or hospice setting
Current CPR certification.
Excellent interpersonal and communication skills
Skill in the use of EMR (Electronic Medical Record), cell phone, text and email systems or the ability to learn.
Must be a licensed driver with an automobile that is insured in accordance with state.
