The LPN/Client Care Manager will impact lives every day by overseeing administration of medication and treatments, managing clients' plans of care, and working with Direct Support staff to ensure proper delivery of care.

Responsibilities of the LPN Client Care Manager:
Assist RN with ensuring safe and proper medication administration and treatments are being provided in Regions 3, 2, and 9 (Thibodaux, Baton Rouge, & Hammond areas). Participate in client assessments as needed.
Track ongoing staff training and competencies to ensure that DSWs have met all requirements to perform non-complex tasks and administer medications safely within the guidelines of LDH, Medicaid, LSBN, and LHCL.
Schedule and track clients’ doctor appointments and communicate scheduling information to other departments. Arrange for transportation as needed.
Log received medications, check for accuracy, and coordinate home delivery in a timely manner.
Review Medication Administration Records (MARs) and medical delegations (MedDels) for completion, accuracy, and signatures. Prepare and maintain client medical binders and records.
Manage care for approximately 25 clients who participate in the following programs:
Medicaid Waivers including SIL, NOW, LTPCS, PAS, SW, CCW, CC/EPSDT
Private Care including long-term care insurance plans
VA Care through the Department of Veterans Affairs
Ensure person-centered care plans are carried out by Direct Support Workers (DSWs) and that clients' health and well-being are maintained.
Communicate frequently with clients, families, and other affiliates to answer questions, resolve issues, and ensure satisfaction with services.
Travel to clients' homes for meetings and visits quarterly, annually, and as needed.
Properly document all client communications, care plan or medical changes, visit notes, or incidents.
Comply with all program guidelines and meet requirements established by LDH, Medicaid, LSBN, and LHCL.
Promote LHCL in the local community and build relationships with external stakeholders
Participate in after hours on-call rotation and serve as backup staff working in-home with clients as needed
Model the LHCL values of Compassion, Trust, Integrity, and Accountability
Must have a valid Louisiana LPN License.
2 years' experience preferred (patient care / medication management / case management)
Must possess strong Administrative and Clinical Skills
Effective Communicator and Educator
Basic computer skills including Microsoft Office, Word, and Excel
Must be able to accept 24/7 on-call responsibility to assure the services provided by LHCL are of the highest quality attainable.
Satisfactorily pass all required and applicable State & Federal eligibility verifications including background, driving and other checks
Valid driver's license, auto insurance, and reliable transportation
Office hours are 8:30 a.m. - 4:30 p.m. Monday through Thursday and 8:30 a.m. - 4:00 p.m. Friday.
