Position Overview: The Care Transitions Nurse is charged with ensuring that MassHealth members and applicants in nursing facilities receive the information and care planning supports to make supported decisions about their future care plans and residential settings. The CSSM program is intended to provide the support necessary to consumers and nursing facility discharge planners to ensure that consumers returning to the community receive appropriate care and supports for a successful discharge.
Position Responsibilities: Responsible for timely completion of nursing facility clinical determination activities, including visiting all consumers within 5 days of the receipt of a referral; weekly onsite comprehensive assessments for the purpose of reviewing clinical data, meeting with all pertinent nursing facility staff, meeting and assessing face to face with the consumer.
Qualifications: •Currently licensed to practice as a Registered Nurse in Massachusetts, with license in good standing required •Three (3) years of clinical nursing experience, with at least one (1) year in community or home health. •Must demonstrate a passion for supporting consumers in achieving their personal goals related to wellness and community living •The ability to appropriately handle confidential and personal information •Proficient computer skills •Expert time manager •Ability to process information accurately •Reliable transportation and valid driver license to make home visits throughout the GLSS service area •Bilingual candidates will be given priority consideration
Hours: Full time- 40 hours per week
