General Summary

The Program Manager is responsible for oversight and operations of a cluster of residential homes for individuals with intellectual disabilities, and/or autism. This person will perform all the duties of program specialist as outlined in 55 PA Code Chapter 6400 regulations and will work to ensure a healthy emotional and physical environment for the assigned residents while assuring compliance with all applicable regulations and policies. The program manager must be able to work independently, be flexible (weekend and overnights may be required), motivate others and exercise good and timely decision making. The program manager must be willing and able to provide direct support services to residents when necessary.

The Program manager will work at the direction of the Program Director and in the absence of the Program Director and be responsible for supervising residential supervisors as well as direct support professionals (DSP).

Essential Duties and Responsibilities

Communicates and fosters company values and philosophies.

Assists the Program Director or designee with general program management.

Serves as the primary contact between Greater Hearts Human Services, administrative entities, support coordination units and other team members; including the individuals supported and their representatives.

Participates in referral and admission process for new individuals

Supervises Residential Supervisors and Direct Support Professionals working in the assigned programs. Manages performance of direct support staff including provision of feedback, development, training, and performance evaluations.

Monitors and evaluates performance of staff according to the company’s performance management policies and procedures.

Monitors program and staff compliance to all regulatory standards and company policies and procedures

Assists HR manager in recruiting, interviewing, hiring, orienting, and training residential supervisors and direct support professionals.

Participates in multidisciplinary team and management meetings including Human Rights Team meetings, ISP meetings etc.

Coordinate schedule for assigned employees and manage coverage in the homes in accordance to programmatic requirements and guidelines.

Complete reviews timesheet and payroll for assigned employees in a timely manner.

Recommends and takes corrective action to ensure quality work performance of immediate employees.

Directs and ensures effectiveness of program’s daily operations.

Ensures the individuals’ health, safety, and welfare.

Responds to incidents and perform incident management duties such as assuring safety, reporting and entering incidents as outlined in the Incident Management Bulletin and company policy on Incident Management.

Observes and supervises employee medication administration training and practices.

Ensures individual health and medical needs are met and updates individuals’ Lifetime Medical History on an annual basis.

Ensures that residential homes and facility work environments are safe, well maintained and free from hazards, as applicable.

Participates in the company’s on-call system.

Enhances quality of the individuals’ lives by promoting inclusion, recovery and integration into the community.

Provides leadership to the team working directly with the individuals by

Providing direction on implementing principles of Everyday lives such as normalization, positive approaches, and self-determination, etc.

Serving as a resource and role model for staff and individuals supported

Facilitates regular staff meetings and ensures communication with and from company management.

Coordinates and completes assessments such as initial / annual assessments and Health risk screening tools (HRST).

Coordinates and/or participates in individual program planning process as well as individual support plan development and reviews in accordance with applicable regulations and company policies.

Ensures implementation of individual support plan and behavioral support plans.

Makes recommendations to revise a service or outcome in the individual support plan based on available data and observations.

Supports the integration of individuals in the community.

Supports individual’s communication and involvement with families and friends.

Oversees the maintenance and completion of documentation required by the company, state or federal regulations, etc.

Transports individuals in personal or company vehicles for meetings, appointments or community activities.

Serves as an active member of the ISP team in the coordination of services for assigned individuals.

Participates in monitoring with Support Coordination agencies.

Develops and maintains cooperative relationships with family members, county and state personnel, other service providers, medical professionals, etc.

Assists in data collection to report and monitor program activities and analyzes data to determine if the activities provided are meeting program outcomes.

Accounts for the management of program budgets and petty cash as applicable to the assigned programs.

Maintains responsibility for all financial transactions on behalf of Greater Hearts Human Services and the assigned individuals and follows all policies and procedures related to financial requirements. Reviews and verifies financial documents such as payroll, expense forms and cash-on-hand forms.

Oversees and maintains records pertaining to inventory, both for company owned property and possessions belonging to individuals, as applicable to the assigned program.

Keeps accurate and carefully prepared records related to client development and medical concerns, staff training follow-up, individual finances, household, and emergency related issues. Ensures that subordinates document accurately and timely.

Develops a suitable framework which provides direct support staff members with the necessary information, training, and supervision so that they can effectively perform their duties in the required manner.

Maintains positive relationships with individuals on caseload, including informing them of company / community activities and checking in on their satisfaction with services, notifying them of schedules and schedule/staff changes.

Document monthly ‘touch base’ meetings with each assigned residents and staff including informing of schedules, staffing changes, activities, service satisfaction reviews, and other pertinent and timely information.

Complete quarterly ISP review reports for each assigned resident and review reports with resident as well as ISP team

On call for all client needs

Monitor fiscal, medical, personal and ISP documentation for each assigned individual to ensure accuracy and compliance.

Complete monthly reports and oversee daily documentations completed by the staff. Submit monthly written reports to the appropriate individuals outlining operational status, progress, and concerns.

Performs job responsibilities of the residential supervisors and DSP directly when required.

Provides coverage in emergency situations.

Accepts appropriate feedback in good faith and in a professional, courteous, and respectful manner.

Makes independent decisions regarding program operations within the scope of his/her authority.

Performs other duties as assigned.
