Temporary Traditional Adult/Older Adult Case Manager

Program Information:
Traditional Adult Mental Health program provides outpatient services such as individual or family and/or group therapy, case management services, and medication support services to adults ages 18-59, who are facing mental health challenges, in regaining hope, developing self determination, and taking personal responsibility.
Traditional Older Adult Mental Health program provides outpatient services such as individual or family and/or group therapy, case management services, and medication support services to assist clients age 60 and older, who are facing mental health challenges, in regaining hope, developing self determination, and taking personal responsibility.

Starting Salary: $18.82/per hour

Second Language and/or Written Differential: Upon passing the required competency test employees will receive, $46.15 per bi-weekly pay period for spoken & $18.46/per bi-weekly pay period for written capability of required Second Language.

Special Requirements: Bachelor’s degree required (employees hired prior to the ratification date of the revised job description are not subject to this requirement) plus one year related experience working with Mental Health. Experience working with adults, older adults required and substance abuse preferred. Knowledge of South County Community resources & ability to collaborate w/ outside agencies including but not limited to county/public, IMDs and Emergency Psychiatric Services (EPS) preferred. Provide intakes, direct services and assessments, must maintain documentation and develop case plans with individuals. Ability to exercise good judgment and act calmly in an emergency situations. Work effectively in a team environment or independently. Demonstrated ability of cultural competency working effectively with people differing in value systems, ethnicities, cultural background, language capabilities and disabilities. Experience and ability to work in a fast paced environment with good time management skills with the ability to maintain appropriate documentation is required. Strong computer, written and verbal skills required. Experience with County Mental Health paperwork and procedures strongly preferred. Knowledge and experience working with the DSM IV and setting therapeutic goals. Must be willing to work with clients in a clinical setting, in the community and/or in clients home. Community-based work to include home visits. Program/client base includes Gilroy, San Jose and/or Morgan Hill. Knowledge of psychiatric medications a plus. Flexible schedule, may be required to work some evenings and/or weekends. Daily travel is required which may include transportation of clients. Must possess or have the ability to obtain a valid California Drivers License, auto insurance, and automobile. Bi-lingual (English/Spanish) required.
