FSP Adult/Older Adult Program Coordinator

Program Information:

Adult Traditional Medication support, individual therapy, group therapy, and case management for seriously mentally ill adults, 18-59yrs.

Older Adult Traditional Medication support, individual therapy, group therapy, and case management for seriously mentally ill adults age 60 or older.
Position Information/Requirements:

Starting Salary:Step 1-$20.68/per hour

Second Language and/or Written Differential:Upon passing the required competency test employees will receive, $46.15 per bi-weekly pay period for spoken & $18.46/per bi-weekly pay period for written capability of required Second Language.

Special Requirements: Bachelor's degree and 2 years related experience or equivalent. Must have at least 2 years experience working with youth and adults. Ability to collaborate and work with hospitals, residential settings, Board and Care facilities and county agencies. Ability to provide case management and crisis intervention services. Ability to provide leadership and direction to the FSP team; facilitate team meetings and work effectively within the community. Must be willing to work with clients in clinical setting, community and/or in clients' home. Demonstrated ability of cultural competency by effectively working with people differing in value systems, ethnicity, disabilities, language capabilities and cultural backgrounds. Maintain a client case load, develop and implement service/treatment plans, and perform intensive services through a variety of modalities. Ability to coordinate multiple daily programmatic responsibilities, complete and submit reports as required, facilitate and participate in county-wide and agency coordinator meetings. Ability to exercise good judgment and act calmly in an emergency situation. Experience and ability to work in a fast paced environment with good time management skills with the ability to maintain appropriate documentation is required. Work effectively in a team environment or independently. Client base Gilroy, Morgan Hill and/or San Jose. Strong computer, written and verbal skills required. Flexible schedule may be required to work some evenings and/or weekends. Daily travel required which may include transportation of clients. Must possess or have the ability to obtain a valid California Drivers License, auto insurance and automobile required.Bi-lingual, Spanish speaking required.
