The Colorado Coalition for the Homeless seeks a Nurse Practitioner/Physician Assistant to work collaboratively with a multi-disciplinary Health Care for the Homeless team. This position is responsible for providing primary health care to individuals and families at the Fort Lyon Health Center. This position works to provide the highest possible quality of primary care, which includes acute and chronic care across the full age span, collaboratively with the FLHC medical/behavioral team. This position is located in Las Animas, CO 81054 and reports to the Medical Director.

NOTE: For compliance reasons, after a job offer has been extended, the candidate chosen to fill this clinical position will be required to complete our agency’s credentialing and privileging process prior to start date. This process can take up to four weeks to complete. Please note that start date will be scheduled once credentialing and privileging is complete.

Qualified candidates are eligible for the National Health Service Corps (NHSC) Loan Repayment Program. Licensed health care providers may earn up to $50,000 toward student loans in exchange for a two-year commitment at the FLHC, an NHSC-approved site.

Essential Job Functions

  1. Provides direct, high-quality, primary medical care to FLHC patients; maintains and enhances a current level of knowledge relative to professional practice, and continually strives to improve the quality of care at FLHC.
  2. Collaborates with other members of the multidisciplinary healthcare team to identify and address the complex issues which may adversely impact the health of homeless clients.
  3. Participates as an effective team member in the provision of medical and healthcare.
  4. Facilitates client participation in self-care by providing information and choices in a non-judgmental, respectful manner.
  5. Takes initiative when appropriate to determine ways to deliver health care in a more compassionate, expedited, user-friendly manner.
  6. Maximizes community linkages while acting as an advocate for patients.
  7. Maintains accurate, legible, and confidential records, and follows HIPPA regulations.
  8. Follows CCH policy regarding supervision of NP/PA staff and chart review/co-signs.
  9. Helps to recruit, train, support, and provide evaluation feedback of the medical staff as needed.
  10. Completes all credentialing and privileging requirements of CCH.
  11. Participates in Quality Improvement and Peer Review activities to provide on-going quality assurance and quality improvements of health services.
  12. Maintains Continuing Medical Education and other requirements to maintain licensures and board certifications.
  13. Maintains Electronic Health Records core competencies as evidenced by:
  • Uses EHR to order point of care tests, procedures, and labs with medically necessary codes.
  • Uses EHR to order patient medications and maintains an up to date list of all medications in use by each patient.
  • Reviews, sign-off and create communication to the patient for results of all diagnostic orders per timeliness policies.
  • Manages tasks, patient communications, medication refill requests in the inbox in a timely manner.
  • Uses the Telephone/communication template to record all non-face to face clinical communications regarding patient's care.
  • Records all patient visits electronically in the HR, ensuring that PMH, PSH, FH, Social History, chronic/active problem lists, allergies, and medications are recorded & updated accurately for each patient.
  • Completes charting and submits all billing codes for patients seen in the clinic within a 48 hour period of time.
  • Records patient information as instructed in ongoing training to capture information needed for revenue compliance, regulatory compliance, and quality assurance.
  • Shares coverage of after-hours critical lab review and response.

Qualifications Summary

Requirements for Nurse Practitioner:

  1. A Master of Science or Doctorate degree in Nursing from a program accredited by the American Nurse Association or the American Association of Nurse Practitioners.
  2. Current registration in the State of Colorado as an Advanced Practice Nurse.
  3. Certification as a Nurse Practitioner from the American Nurse's Credentialing Center.
  4. Minimum of 3 years experience with outpatient management of complex adult medical problems. 5 years preferred.

Requirements for Physician Assistant:

  1. Graduation from a program accredited by the American Academy of Physician Assistants.
  2. Current licensure with the State of Colorado to practice as a Physician Assistant.
  3. Current certification with the National Commission of Certification of Physician Assistants.
  4. Minumum of 3 years experience with outpatient management of complex adult medical problems. 5 years preferred.

Our History
CCH was established in 1984 as a 501(c)(3) non-profit corporation. It was founded by a group of individuals with a will to take action on behalf of Colorado's homeless. They recognized the struggles of working people living in poverty. And they refused to tolerate the dangers faced by those people sleeping in their cars, in tents, or on the streets.

More than 30 years later, many of the underlying social causes of homelessness persist, like poverty, shortages of affordable housing, and unattainable healthcare. The Coalition's integrated housing, healthcare, and supportive service programs have been highly effective at responding to these conditions over time.
