The Colorado Coalition for the Homeless seeks a Family Support Program Manager to oversee a team of FSS Case Managers and Clinical Case Managers to ensure appropriate and high-quality mental health and case management services are provided to families participating in FSS (Family Support Services) housing programs in accordance with CCH and regulatory guidelines. This position is located at 2111 Champa Street, Denver, CO 80205 and reports to the Family Support Services Director.

NOTE: For compliance reasons, after a job offer has been extended, the candidate chosen to fill this clinical position will be required to complete our agency’s credentialing and privileging process prior to start date. This process can take up to four weeks to complete. Please note that start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions

  1. Provides ongoing supervision to FSS case managers and clinical case managers to ensure staff follow acceptable standards, complete work in a timely and accurate manner within the CCH regulatory guidelines. This specifically involves a review of the electronic health record, treatment plans, case plans, Homeless Management Information System (HMIS) case notes, and other documentation within case files.
  2. Provides individual and family/group therapy, crisis intervention and makes referrals to other mental health providers for FSS participants as appropriate.
  3. Completes required paperwork and ensures accurate and timely reporting to Regional Accountability Entities (RAEs), (formerly BHOs), CCH administration and funders. Also ensures supervisees meet all reporting requirements.
  4. Works to establish community collaboration promoting effective coordination and continuity of services to families participating in the FSS program, particularly those participating in housing programs. Close collaboration is particularly important with local schools, community agencies, vocational services, etc.
  5. Participates in coordinated meetings with RPMC teams at various property sites throughout the Denver metro area, and serves as communication bridge between program and property.
  6. Works collaboratively with RPMC regarding community building relations, activities, and events at assigned properties.
  7. Recruits and hires clinical and case management staff with the cooperation of the FSS Team and completes performance reviews.
  8. Oversees interns and volunteers when applicable.
  9. Assists with grants, Annual Progress Reports (APR’s) and ensures that program data is collected efficiently and effectively so that it can be used in APR’s and other program reports.
  10. Monitors the budget, reviews and assists with variance reports and check requests.
  11. Coordinates services with Stout Street Health Center, especially psychiatry, pediatric and mental health services and assists other CCH programs regarding family-related issues.
  12. Demonstrates core competency in HMIS and EHR medical records system.
  13. Carries a cell phone to assure accessibility and responsiveness to both emergent and non-emergent issues.
  14. Maintains a work environment that is marked by respect for others; that values inclusiveness and builds workforce diversity, and that fosters cooperation and teamwork.
  15. Performs other duties as assigned by management.

Qualifications Summary

  1. Licensed Clinical Social Worker (LCSW) in the State of Colorado with two to four years management experience. Preference is given to candidates with specialized knowledge of issues pertaining to families and children.
  2. Experience in providing clinical supervision and working with a multi-disciplinary team.
  3. Bilingual (English/Spanish) preferred.

Our History
CCH was established in 1984 as a 501(c)(3) non-profit corporation. It was founded by a group of individuals with a will to take action on behalf of Colorado's homeless. They recognized the struggles of working people living in poverty. And they refused to tolerate the dangers faced by those people sleeping in their cars, in tents, or on the streets.

More than 30 years later, many of the underlying social causes of homelessness persist, like poverty, shortages of affordable housing, and unattainable healthcare. The Coalition's integrated housing, healthcare, and supportive service programs have been highly effective at responding to these conditions over time.
