The Colorado Coalition for the Homeless seeks a Clinical Pharmacist to provide chronic disease management services in an integrated health setting, including collaboration with other health professionals, managing patient drug therapy, provide patient education and promoting high quality, evidence-based, cost-effective care. This position is located at 2130 Stout Street Denver, CO 80205 and reports to the Director of Pharmacy.

NOTE: For compliance reasons, after a job offer has been extended, the candidate chosen to fill this clinical position will be required to complete our agency’s credentialing and privileging process prior to start date. This process can take up to four weeks to complete. Please note that start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions

  1. Evaluates drug therapy through clinical assessment, direct patient care involvement, subjective and objective findings relating to patient’s responses to drug therapy
  2. Communicates and documents findings and recommendations to providers, care teams, and patients
  3. Assures optimal and effective therapeutic plans in utilizing the highest quality, evidence-based and cost-effective medications for our patients
  4. Provides patient and healthcare professional education and medication information
  5. Performs medication therapy management and comprehensive medication review upon referrals from care team members
  6. Participates in agency-wide process of assessing, managing, preventing and reporting of drug interactions, medication errors, and/or adverse drug reactions
  7. Participates in the development of medication use management programs including clinical guidelines, clinical decision pathways, disease state management and drug use programs in cooperation with Medical Director and Director of Pharmacy
  8. Serves as clinical preceptor for doctoral pharmacy students
  9. Identifies optimal and individualized solutions to overcome medication adherence issues
  10. Provides tele-pharmacy services to patients in Fort Lyon Health Center

Qualifications Summary

  1. Doctorate of Pharmacy Degree required
  2. Current licensure in the State of Colorado as a registered pharmacist in good standing with the Colorado State Board of Pharmacy.
  3. Prior clinical pharmacy experience is required
  4. Completion of ASHP Residency PGY-1 is required
  5. Board Certification in Pharmacotherapy is strongly preferred
  6. Completion of PGY-2 residency with ambulatory care focus is strongly preferred
  7. Bilingual (English/Spanish) preferred.
  8. Able to pass automobile insurance carrier’s motor vehicle record check.

The mission of The Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.

Our History
CCH was established in 1984 as a 501(c)(3) non-profit corporation. It was founded by a group of individuals with a will to take action on behalf of Colorado's homeless. They recognized the struggles of working people living in poverty. And they refused to tolerate the dangers faced by those people sleeping in their cars, in tents, or on the streets.

More than 30 years later, many of the underlying social causes of homelessness persist, like poverty, shortages of affordable housing, and unattainable healthcare. The Coalition's integrated housing, healthcare, and supportive service programs have been highly effective at responding to these conditions over time.
