POSITION SUMMARY: The Community Relations Liaison Assistant in conjunction with the other members of the Admission Team, is responsible for activities and actions related to promoting the services and expertise of the organization to the medical community and to patients and family members. This is an entry level position for candidates that are seeking to build a career in healthcare relations.
QUALIFICATIONS: Education: Bachelors degree in health care related field. Credentials: None Experience: At least two years of health care experience. At least one year of hospice or home health experience. Core Competencies: Knowledge of company requirements including Medicare, Medicaid, and private insurance guidelines. Good communication and marketing skills. Ability to respond to the needs of physicians, discharge planners, patients and family members. Ability to make meaningful observations and write comprehensive accurate reports.
FUNCTIONS & RESPONSIBLITIES: 1. Work in conjunction with the admission team. 2. Goes in to the field and establishes presence of our organization to our service members. 3. Responds to referrals for evaluation and/or admission to program. 4. Verifies and completes all admission and financial information. 5. Explains the program to patients, family members, and other referral sources. 6. Works with admission team for referrals, admissions, and care coordination of patient and family needs. 7. Identifies key referral sources and assists the admission counselors to develop a marketing plan for the program. 8. Attends and participates in in-services, staff meetings and case conferences. 9. Completes all documentation in an accurate and legible manner. 10. Complies with company policies and procedures, laws and regulations and standards of practice. 11. All other tasks and duties deemed necessary and appropriate.
