Assistant Manager of Service Coordination Team
Center for Disability Rights, Inc., Full-Time, Rochester, NY
Supervises Service Coordinator Team in TBI, NHTD, and OPWDD in collaboration with the Manager of Service Coordination. The main goal is to assist the participants in maintaining and obtaining services that will assist them to live in the most integrated setting possible, within the community.
Conduct intake assessments, evaluations, and develop individual service plans
Review and provide feedback to staff regarding ISP’s and ISR’s.
Track referrals to the department
Maintaining a small caseload
Participate in team meetings with participants and their service providers
Understanding of Independent Living philosophy; familiarity of the ADA
Knowledge and understanding of welfare, Social Security benefits and entitlements
Understanding of developmental disabilities, traumatic brain injury, and mental illness
Awareness of the challenges facing individuals with disabilities who choose to live independently
Bilingual English-Spanish and/or American Sign Language preferred
Master degree Minimum of one year experience providing service coordination for individuals with disabilities and knowledge about community resources OR Bachelor’s Degree in a human service related field and two (2) years of experience
At least one year of supervisory experience
