DESCRIPTION

The Community Health Centers have been providing quality primary care and behavioral health services to the citizens of Lane County since 2004. We provide a broad array of services to over 18,000 adults and children annually in an outpatient setting.  The CHC is the primary source for uninsured and underinsured. Our clinics are Federally Qualified Centers whose mission is to serve the community by offering excellent care and eliminating barriers to healthcare.  Our staff must show dedication to providing care with compassion and willingness to work with a diverse population.

Duties include, but are not limited to:  triaging patients, conducting Nurse-Only visits, assisting providers as necessary, training others, monitoring vaccines.

Community Health Nurse 1: As a member of a total health care team, provides entry level professional nursing duties relating to public health and community health care services in health promotion, prevention and disease control programs; and performs related duties as assigned.

Community Health Nurse 2: To provide a variety of professional population-focused community and public health nursing services in health promotion, prevention and disease control programs; and to perform related duties as assigned.

CHN 1: $23.87 – $33.10 / hour  
CHN 2: $26.33 -  $36.54 / hour

As a member of a total health care team, provides entry level professional nursing duties relating to public health and community health care services in health promotion, prevention and disease control programs; and performs related duties as assigned.

Schedule: Hours will vary depending on operational needs.

*This position is budgeted for a maximum of 1,040 hours during the fiscal year beginning July 1st, 2017 and ending June 30, 2018.

QUALIFICATIONS

CHN1:
Assesses, plans and evaluates individual, family and community health needs in consultation with health team.

Implements general public or community health nursing services in an assigned geographic area of responsibility or special program.

Arranges or conducts health classes or group meetings with teachers, school nurses, parents, industrial groups and expectant parents, as assigned.

Performs home or office visits, explaining basic concepts of prevention of disease and promotion of mental and physical health.

Assists families in understanding health needs and methods of meeting needs through education, direct service and referral to appropriate health resources.

Exchanges information with physicians and other health workers for more effective care of the individual patient in consultation with health team.

Schedules delivery of health services including home visits, office visits, groups, clinics, or telephone consultations, as appropriate.

Confers with pupils, teachers, administrators, school nurses, parents and physicians on specific health problems.

Participates in conferences with other disciplines and agencies to coordinate services and plan joint action.

Maintains records for nursing services to families, schools and clinics, and makes reports as required.

Provides public and community health nursing services in special settings such as hospital coordination, disease investigations and mental health and drug programs in consultation with
health team.

Dispenses medication under standing orders of a physician, and supervises and monitors its effects on a patient.

Collects laboratory samples and interprets their results where applicable.

May perform other duties as assigned.

CHN2:
Utilizing independent judgment, assesses, plans and evaluates individual, family and community health needs.

Plans and implements general public or community health nursing services in an assigned geographic area of responsibility or special program.

Plans and directs clinics or special programs with clinics.

Arranges or conducts health classes or group meetings with teachers, school nurses, parents, industrial groups, and expectant parents.

Performs home or office visits, explaining basic concepts of prevention of disease and promotion of mental and physical health.

Assists families in understanding health needs and methods of meeting needs through education, direct service and referral to appropriate health resources.

Plans and exchanges information with physicians and other health workers for more effective care of the individual patient.

Organizes and coordinates the development of community organization and planning for health services with community groups.

Schedules visits with school administrators and teachers in a district and assists them in identifying health problems and interpreting medical reports and recommendations.

Participates in conferences with other disciplines and agencies to coordinate services and plan joint action.

Maintains records for nursing services to families, schools and clinics and makes reports as required.

SUPPLEMENTAL INFORMATION

CHN1:
Training:
Associates degree in Nursing from an accredited college or university is required.

College-level coursework in the social sciences, and additional nursing training within a community or public health curriculum desirable.

Experience:
Nursing experience in public health or community health care services desirable.

An equivalent combination of experience and training that will demonstrate the required knowledge and abilities is qualifying.

CHN2:
Experience and Training:
Bachelor of Science in Nursing degree from an accredited college or university.

An equivalent combination of experience and training that will demonstrate the required knowledge and abilities is qualifying.

Special Requirements:
• Possession of a current State of Oregon Registered Nurse license at time of appointment.
