Job Description


Summary:

Serves as an active part of a team that provides in-home assessment and clinical treatment services
to parents and children who are in the process of reunification. The goals are to assess family’s
readiness for reunification and to provide parenting education and support. This position requires
collaboration with community providers and funders.

Essential Responsibilities and Target Outcomes:

  • Performs comprehensive assessment of family functioning, explores risk and safety concerns, identifies the strengths and needs of families and identifies barriers to reunification.
  • Provides Family Time or Therapeutic Family Time services.
  • Assists with appropriate referrals and linkage to others services, if appropriate.
  • Develops treatment goals and objectives with adult and/or family. Follows Reunification Model and appropriate treatment modalities for the family.
  • Preserves and restores the parent/child attachment, and reduces the child's sense of abandonment and loss.
  • Evaluates client risk on an ongoing basis and formulates and implements appropriate safety plan.
  • Collaborates and coordinates with external agencies such as schools, PCPs, pediatricians, DCF, probation, court and other social service agencies.
  • Communicates with other clinical staff, case manager, DCF, pediatricians, and other members of the provider team and completes documentation within program and CMHA time-frames.
  • Provides crisis intervention to family in the home, office, or community if need arises. This includes on-call 24/7 coverage of families on caseload.
  • Maintains accurate documentation and submits within program and funder timeframes.
  • Maintains appropriate caseload as determined by CMHA and funder.
  • Performs other related duties as assigned.


Principal Customers:  Clients referred by DCF.


Critical Work Relationships:
Regional Director, Coordinator, Coordinator of Home-Based Services, Program Clinicians, Case
Manager, psychiatrist, DCF, Juvenile Court, Schools, Pediatrician, other programs within CMHA and
other Social Service Agencies as appropriate.


Minimum Requirements:

Education:  Master’s Degree in behavioral health (psychology, social work, counseling, marriage and family therapy, alcohol and drug counseling).

Credentials and Licenses:

CT LCSW license preferred. Clinician II must have a Connecticut license as a LCSW, LMFT, LPC,
Ph.D., Psy.D. LADC as appropriate for services. Certified in TCI, First Aid and CPR.

Experience:

  • Successful experience in the provision of substance abuse and mental health services with children, adolescents, adults and/or families, preferably a minimum of two years.
  • Bilingual preferred (English/ Spanish).
  • Beginner to intermediate experience with Microsoft Office products including Word and Outlook. Experience in, or the ability to learn how to use an Electronic Medical Record (EMR) system which includes appointment scheduling, authorization tracking, assessments, treatment plans, progress notes and discharge summaries.
