Job Description
Summary: The Clinical Pharmacist shall provide services in support of the Soldier Centered Medical Home (SCMH) and its mission to reduce poly-pharmacy risks to soldiers. The Clinical Pharmacists shall perform essentially the same duties as those required of any military or government civil service pharmacist. Minimum Qualifications:(To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.) Possess a six-year Doctor of Pharmacy degree obtained from a College of Pharmacy which is accredited by the American Council on Pharmaceutical Education (ACPE). Possess a minimum of two years post-graduate experience as a Clinical Pharmacist. Possess a current, unrestricted license to practice pharmacy in any one of the United States of America, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands. Have and maintain current certification in Basic Life Support (BLS) with Automated External Defibrillators (AED) certification. This must be obtained through the American Heart Association Be able to meet credentials approval of the MTF credentialing committee and be granted privileges by the MTF commander.
