COMPANY PROFILE:
Geriatric primary care practice serving patients in Indianapolis and surrounding communities. Care is provided in patient residences and facility settings. The organization's leadership is committed to fostering a positive, supportive environment - the employment culture is one of mutual respect and clinical autonomy.
POSITION DESCRIPTION:
Serve as a primary care provider for a dedicated base of geriatric patients.
The clinical focus is on preventative medicine, chronic disease management, and geriatric primary care.
You will see your patients as often as you deem necessary, which is typically 1 visit per month.
See 8 to 12 patients per day, conveniently located near each other as scheduled by the office's highly-trained team.
Most days, you will begin and end at your home. Only minimal visits to the office in Indianapolis are required.
You will utilize a mobile EHR on a laptop or tablet.
There are no night, weekend, or on-call responsibilities.
The exact schedule and structure of this position are highly negotiable. The ideal set-up would be 3 1/2 or 4 days of patient care per week, with the remaining 1 to 1 1/2 days per week being "work from home" hours. During these times, the provider would simply need to be available via phone for their coordinator, should anything arise at the office that requires the providers attention.
The territory for this position is growing and, in the near future, this position could convert into a traditional full-time position that consists of 5 days of patient care.
REQUIREMENTS:
- Indiana PA license
- DEA
- previous exposure to geriatric care, home health, or house calls preferred
- at least 1 year of advanced practice experience
