We are now recruiting medical and behavioral health professionals for immediate openings in our new Behavioral Health Community Partners Program (BHCP).

Clinical Care Manager

Under the direction of the Program Director of Care Coordination, the Clinical Care Manager will provide supervision to care coordination staff (i.e. staff trained as community health workers and recovery coaches) and ensure compliance with the BHCP contract responsibilities. Also responsible for care management in collaboration with enrollees, medical and behavioral health providers, social service agencies, and other supports as needed.

Requirements:

• Licensed Certified Social Worker (LCSW), Independently Licensed Social Worker (LICSW), Mental Health Counseling (LMHC), Marriage and Family Therapy (LMFT) or Alcohol and Drug Counseling (LADC I)
• Master’s degree in social work, psychology, counseling or other clinical concentration
• Managerial experience overseeing clinical and non-clinical staff strongly preferred
• Experience providing person-centered care coordination and care management to adults with behavioral health, substance use and/or medically complex issues
• Experience with accessing and knowledge of local resources, navigating health, behavioral health, and addiction treatment systems; Strong familiarity with community based resources and services
• Valid Driver’s License with use of a reliable, properly insured vehicle
• A pre-employment Criminal Offense Record Information (CORI) check is required

Nurse Care Manager

Collaborating with the Care Coordinators and other program staff, the Nurse Care Manager will contribute to medically-oriented aspects of the BHCP services with focus on integration and coordination of care. The nurse will be responsible for reviewing and approving Person-Centered Treatment Plans and will work with medically complex individuals and their families.

Requirements: 

• Registered Nurse with current licensure in Massachusetts
• Minimum of one year of experience in nursing or healthcare within the last five years.
• Experience providing person-centered care coordination and care management to adults with mental illness and substance use disorders.
• Experience with accessing and knowledge of local resources, navigating health, behavioral health, and addiction treatment systems; Strong familiarity with community based resources and services
• Valid Driver’s License with use of a reliable, properly insured vehicle
• A pre-employment Criminal Offense Record Information (CORI) check is required
