Job Description


The Behavioral Health Home managed by Bay Cove Human Services is a program that provides comprehensive care coordination to individuals with chronic conditions, including a behavioral health concerns. The model operated at Bay Cove is the Coordinated Care Model, where staff ensures members are receiving the medical and behavioral health services they need. Bay Cove Health Home staff break down their work into four broad categories: assessment, care planning, authorization management, and care coordination. The Commonwealth of Massachusetts has chosen the insurance agency Commonwealth Care Alliance (CCA) as a Health Home provider. 

We are presently seeking to hire a Licensed Clinician - Health Care Coordinator to:

  • Carry a caseload of members that live in the community
  • Develop person-centered care plan (Integrated Care Plan- ICP) with their members
  • Responsible for care coordination of member's psychiatric and medical needs with all involved community providers and Bay Cove teams
  • Collaborate with member's care team to ensure access to appropriate community resources 

Job Requirements

  • Independently Licensed Clinician; LICSW/LMHC.
  • At least 2 years post-masters experience in relevant field required.
  • Clinical and Case Management skills required
  • Experience with working with substance abuse issues preferred Demonstrated ability to work effectively and collaboratively with a culturally diverse population of clients, other care providers, family and community members.
  • Demonstrated leadership skills Demonstrated ability to work collaboratively as a member of multidisciplinary and cross-functional teams.
  • Demonstrated ability to function as an effective change agent.
  • Demonstrated ability to negotiate and resolve differences Demonstrated ability to function under pressure in a fast paced human service environment Demonstrated ability to identify opportunities and obstacles and develop effective, creative solutions to pursue opportunities.
