Care Coordinator – Assisted Living

Full Time

RN or LPN required.  

 Under limited supervision, is responsible for assessing, implementing, evaluating and documenting resident care in the assisted living units.  Supervises and teaches resident activity coordinators involved in resident care, programming and activities.  Oversees resident care and coordinates, supervises and evaluates resident services to assure that each resident receives services appropriate to the individual’s needs.  Must see to the provision of resident care by assessing, planning, intervention, and implementation of care plans.
