The Social Worker primary responsibility is service to students suspected of having disabilities and students with disabilities; completes initial evaluations and assessments of students suspected of having disabilities and inputs data into the CPS IMPACT SSM System; completes requests for re-evaluations and re-assessments of students with disabilities and inputs data into the CPS IMPACT SSM System; completes triennial re-evaluations and re-assessments of students with disabilities and inputs data into the CPS IMPACT SSM System; collaborates with the Clinician Team and the Special Education Team through the FIE Evaluation Meeting and/or the IEP or 504 Meeting in the recommending student services/interventions; conducts individual or group counseling, as well as provides crisis intervention when needed; assists in the development and implementation of the Functional Behavior Analysis Plan (FAB)/Behavior Intervention Plan (BIP); documents delivery of Related Services through the CPS IMPACT SSM Systems’ Progress Notes; documents delivery of Related Services through the CPS IMPACT SSM Systems’ Progress Notes and acts as a liaison between student/family, Clinician Team, Campus, YCCS and CPS Office of Specialized Services’ Charter Manager to ensure services are provided promptly and effectively.
Qualifications:
· Advanced Degree in Social Work
· Illinois Type 73 Certification
· A minimum of 3-5 years of highly successful related working with students with disabilities
Demonstrated ability to lead the work of others to produce successful and effective outcomes
