Job Description: POSITION SPECIFICATIONS: · Must have a high school diploma, GED or success completion of Woodcrest’s orientation and training program for unlicensed staff and has been determined competent to provide services and follow Woodcrest’s procedures and policies by the RN. · Be over the age of 18, to use a Pal- Lift. · Adaptability and flexibility in dealing with changing situations and can remain calm when interacting with the residents and their families. · Ability to demonstrate good judgment and observation skills. · Ability to effectively communicate with residents, their family members, staff, and to be able to exhibit good customer service skills always. · Ability to read, write, comprehend, and follow verbal and written instructions including medical terminology. · Ability to work independently and to use good problem- solving skills. · Ability to meet the physical demands of the position. · Ability to work flexible times including weekends, evenings, and holidays. · Experience in working with older adults. Previous experience in a hospital, nursing home, or home care agency preferred. · Must be dependable and punctual with a good attendance record. · must follow building dress code and maintain good personal hygiene and grooming. · must be able to prioritize an organized work effectively and efficiently. · Must work as a team member with other personnel. OTHER REQUIREMENTS: · Must receive a not disqualified criminal background result from Department of Human Services and may not be on the OIG exclusions list. · must be screened for TB as well as a second step one. · high school diploma or GED preferred. · passing a drug test upon higher if owner requests one. DUTIES AND RESPONSIBILITES: · Assist with residence with personal cares as identified in the assessment/ sheet resident care plan. these cares may include all or some of the following : assisting with dressing, grooming and oral cares, Shampooing of her, assisting with showering or bathing, rubbing with lotions, fingernail care an foot care if instructed to do so by your RN, re direction of a confused client and other cares as assigned. · Assisting the resident in the use of the commode, urinal, toilet and with adult incontinence product as assigned. · Assisting with ambulation and transfers, including proper use of a transfer belt and assistive devices an proper body mechanics to avoid injury. · Assistant use of hearing aids and glasses for clients. · Medication reminders, assistance, or administration as delegated by your RN. · Preparing meals as directed, following special meal plans if instructed. · Taken record temperature, blood pressures, pulse and respiration as instructed. Report changes or concerns to your RN. · notify your RN case manager/ director/supervisor of any complaints or concerns raised by the resident or the resident’s family or representative. · Help each resident to reach and maintain his or her highest level of performance an independence. Must promote resident’s independence, not dependence, by offering choices and fostering self-help skills. · Establish a relationship with resident, lead resident in activities as assigned and provide socialization and companionship to residents while performing assigned tasks. · Assist with treatments as delegated by the RN according to the residents individualized care plan or with therapy tasks as assigned by a licensed health professional. · Observe residence and report to the LPN RN case manager in changes of the residents physical and emotional condition, such as changing an attitude appearance any behavioral issues, loss of appetite, weight, etc. · assist coworkers with daily responsibilities when needed. · Organize and prioritize assigned work during scheduled shift to complete assigned tasks. · Is responsible for making sure the resident has all supplies and proper equipment with them to complete the assigned tasks. · Document services provided accurately and consistent with Woodcrest policy and complete all required charting in a timely manner. · Attend required in service training at least 8 hours for every 12 months of employment as well as staff meetings and other Woodcrest events. · Maintain a clean environment for the residence. Dust, vacuum, mop, and other household chores as scheduled and assigned. · Assist with transferring, blood glucose monitoring, insulin injections, eyedrops, nasal sprays, NAB treatments and medication administration. · Use validation techniques and or appropriate diversion activities when working with residents who have dementia. · follow Woodcrest dress code and maintain good personal hygiene and grooming. · Follow Woodcrest policies regarding safety rules and requirements and report any issues or concerns about your safety for any injuries immediately to the RN case manager, director/ supervisor. WORKING CONDITIONS: · Primary responsibilities will be conducted at Woodcrest. · Exposure to bodily fluids, infection, orders a behavior of residents that may be aggressive, combative, or demanding. · The noise is usually mild in the house. · exposure to chemical and respiratory hazards is possible. PHYSICAL DEMANDS: The physical demands described here must be met by an employee to successfully perform the essential functions of this job.
