The Referral Coordinator is an office-based position that is part of the team that makes up the Community Support for Families Program in the Hartford/Manchester region (office at 30 Arbor Street, Hartford). The Referral Coordinator works closely with DCF, Program Supervisors and partner agencies to receive and distribute referrals to the Community Support for Families programs according to case assignment parameters and within the required time lines established for the program. The Referral Coordinator has the responsibility for the organized and efficient flow of cases to staff in the program and for assessment and data entry that are essential to program success. The Referral Coordinator also develops and maintains a data system to monitor and track program performance and completes follow-up phone surveys with families.
EDUCATION AND EXPERIENCE/QUALIFICATIONS
BA/BS degree in Psychology, Social Work, Marriage and Family Therapy, Counseling or related field is required. Position also requires strong organizational skills, data management (data entry, development of spreadsheets, and development of data reports for stakeholders), ability to work as a member of a team utilizing a strengths-based perspective and effective verbal and written communication skills.

Receives, reviews, and assigns referrals to the Community Support for Families program according to established case-assignment parameters.
Logs referrals and assures response within contracted time frames.
Maintains accurate utilization data and tracking of available resources to facilitate efficient case assignments.
Maintains, summarizes and prepares reports regarding program performance data and distributes to relevant stakeholders according specified frequency and established deadlines.
Communicates effectively with staff, referral sources, community partners and families.
Adopts a friendly customer service orientation to promote stakeholder satisfaction with services.
Develops, administers and summarizes findings from a family follow-up phone survey to assess program effectiveness/impact.
Participates in staff meetings, meetings with funders and/or community partners to support effective program implementation and timely engagement of families in services. Contributes to discussions regarding resource utilization, family needs and other program performance indicators.
Supports daily program specific operational functions as required.
Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
