Job Description The Outreach Case Manager - Rural Health engages in outreach efforts directed toward the migratory and seasonal agricultural worker populations to connect them with the services of Westside Family Healthcare and its collaborating partners. During the spring and summer, the Outreach Case Manager - Rural Health travels to farms, labor camps and other housing locations within Kent and Sussex Counties to address the health and social service needs of the migratory and seasonal agricultural workers, act as a liaison between them and social services agencies, and participate in health fairs as a lay health educator. Core Responsibilities: 1. Develop and maintain relationships with farm owners, managers and crew leaders to gain access to migratory and seasonal agricultural workers and update crew, farm, agricultural worker housing maps and census data within the service areas 2.
Register all migratory and seasonal agricultural workers and document all services via a Social Services Encounter 3. Utilize designated assessment tool for assessing the overall health needs of the agricultural worker community 4. Develop, organize, translate and produce culturally sensitive and competent health education materials needed for agricultural worker outreach and educational events geared towards the targeted population and participate in health education activities 5.
Travel to farms on a weekly basis to provide social service resources, health education and follow-up to migratory and seasonal agricultural workers 6. Recruit health education class participants through agricultural worker community outreach 7. Attend community meetings and participate in community events/fairs as a lay health educator 8.
Maintain records such as attendance and health education logs as required 9. Maintain an inventory of outreach supplies and place orders for additional supplies as needed with approval from the Director of Rural Health Outreach 10. Drive the WFH van to transport patients safely to and from appointments at WFH and the lab MINIMUM OBJECTIVE QUALIFICATIONS 1.
High school diploma or GED 2. Two years of related experience, such as community services, the healthcare environment or social services work 3. Fluency in written and spoken English and Spanish, or English and another language spoken by migratory and seasonal agricultural workers working in Delaware 4.
Six months of experience using computers and Microsoft Office software 5. Driver’s license and clean driving record PREFERRED OBJECTIVE QUALIFICATIONS 1. Associate’s or Bachelor’s degree in education, business, psychology or another health-related field 2.
Experience in providing services to seasonal and/or migratory agricultural workers 3. Basic life support certification
