Westside Family Healthcare is seeking a full-time Family Nurse Practitioner (FNP) to work as part of an interdisciplinary team providing direct patient services with an emphasis on prevention and education, managing acute and chronic conditions for patients across the lifespan. Westside is a Joint Commission accredited Federally Qualified Health Center, certified as a Primary Care Medical Home, with several locations throughout Delaware.

Our Family Nurse Practitioners play a vital role in caring for our communities, collaborate with the clinical team of providers and allied health professionals, and participate in quality improvement activities.
Responsibilities:
Analyze and interpret patients' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses and treatment plans based on scientific rationale, standards of care, and professional practice guidelines
Recommend or implement diagnostic or therapeutic interventions with attention to safety, cost, invasiveness, simplicity, acceptability, adherence, and efficacy
Perform routine or annual physical examinations and primary care procedures such as suturing, splinting, administering immunizations, taking cultures, and debriding wounds
Prescribe medications based on efficacy, safety, and cost as legally authorized, including medication dosages, routes, and frequencies based on patient characteristics such as age and gender and counsel patients about drug regimens and possible side effects or interactions with other substances such as food supplements, over-the-counter (OTC) medications, or herbal remedies
Detect and respond to adverse drug reactions, with special attention to vulnerable populations such as infants, children, pregnant and lactating women, or older adults
Educate and engage patients in self-management of acute or chronic illnesses, tailoring instructions to patients' individual circumstances
Order, perform, or interpret the results of diagnostic tests such as complete blood counts (CBCs)
Maintain timely, complete and detailed records of patients’ personal health information in the electronic medical records system and keep such information confidential in accordance with legal requirements
Keep abreast of developments in nursing, medicine and regulatory processes and payer systems such as Medicare, Medicaid, managed care, and private sources, maintaining competence in clinical
practice with participation in continuing educational experiences and maintenance of Basic Life Support certification
Perform all services in compliance with the most current, accepted professional standards and the standards of the Joint Commission
Consult with or refer patients to appropriate specialists when conditions exceed the scope of practice or expertise
Maintain organizational policies and procedures in areas such as safety and infection control
Provide emergency medical treatment to WFH patients as needed, and provide emergency medical treatment as requested or appropriate to non-WFH patients who are at or near a location where the FNP is present for the purpose of providing services to WFH patients.
Provide clinical education and training of staff and students
Participate in WFH performance improvement activities and community education and outreach programs
MINIMUM OBJECTIVE QUALIFICATIONS
1. Graduate of an accredited school or college of nursing with a master’s degree in nursing
2. Current registered nurse licensure and advanced registered nurse practitioner licensure and certification
3. Clinical training and experience in ambulatory care
4. Basic life support certification
5. Ability to travel between worksites as needed
PREFERRED OBJECTIVE QUALIFICATIONS
1. Clinical training and experience in ambulatory care in a community healthcare setting
2. Fluency in written and spoken English and in spoken Spanish, including medical terminology
