Functions as a member of the multidisciplinary team and provides treatment rehabilitation and support services.ESSENTIAL DUTIES AND RESPONSIBILITIESProvide direct clinical services to clients on an individual, group, and family basis in the office and in the community settings to teach behavioral symptom-management techniques to alleviate and manage symptoms not reduced with medication and to promote personal growth and development by assisting clients to adapt and cope with internal and external stressorsProvide case management for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team; assume secondary responsibility for developing, writing, implementing, evaluating and revising over-all treatment goals and plans in conjunction with ITT; ensuring immediate changes are made in the treatment plans as clients' needs change; educate and support clients' families, and advocate for clients' rights and preferences.Complete clinical assessments that describe the needs and levels of functioning for the client in a number of specific areas for the determination of servicesAct as liaison and consult with community agencies and families to maintain coordination in the treatment processProvide on-call crisis intervention covering nighttime hours and serve as a backup to evening and weekend staffComplete timely documentation of client progress towards goals and client activity utilizing an established electronic medical records system. Complete all required paperwork in compliance with the VA contract, state standards, and WMMHC Policies and ProceduresProvide vocational counseling to assist clients in meeting employment or educational goalsContribute to daily team meetings and treatment team meetings as necessaryProvide ongoing assessment, problem solving, side-by-side services, skill training, supervision, and environmental adaptations to assist clients with activities of daily livingAssist and support clients to carry out personal hygiene and grooming tasksProvide nutrition education, meal planning, grocery shopping, and food preparationUnder direction of nursing staff, assist clients with their medications through phone prompts, in-person prompts, and utilizing medication lockboxesTravel to other communities as needed for client care according to State standards and treatment plansThis position requires 100% dedication to PACT/MACT program.Additional duties as assignedEDUCATION AND EXPERIENCEMaster's Degree in Social Work, Rehabilitation Counseling, or PsychologyLCSW, LCPC, or licensed psychologistOne year of progressively responsible post graduate experience in a community health center or similar settingCERTIFICATES, LICENSES, REGISTRATIONSLCSW, LCPC, or licensed psychologistCompletion of CPI (Crisis Prevention Intervention), First Aid, CPR training requiredAbility to pass background check and driver's license check upon offer of emp
