Position Summary:Under general supervision of the Care Coordinator Manager, provides intensive case management services within a team setting to a group of individuals with severe and persistent mental illness.Essential Duties and Responsibilities:Assists consumers to identify strengths needs and goals in residential, health, vocational, educational, social and economic domains.Coordinates necessary specialized services including psychiatric, psychological, medical, educational and housing based on the needs and desires of the consumer.Involves family and significant others, with the appropriate releases, as needed in the assessment and planning process of service delivery.Assists in the development and implementation of written case management treatment plans to be signed by both the treatment team.Facilitates consumer access and utilization of natural community resources.Provides transportation to consumers using a motor vehicle, this typically requires a current Montana... driver's license and the ability to operate a motor vehicle.Participate in ongoing supervision with supervisorProvides services to consumers in environments which are preferred by the consumer.Provides outreach as a preferred mode of service delivery.Accompanies consumers to services and resources in the community as needed and desired by the consumer.Acts as an advocate on behalf of the consumer and insure the protection of rights through direct intervention, education and facilitation of advocacy groups in the community.Utilizes supportive counseling as a means to assist consumers to set realistic, obtainable goals; to offer support and assist consumers in the decision making process.Assists consumers to acquire life skill education in the natural environment in order to maximize an independent lifestyle.Provides and coordinate emergency services, including the deployment of crisis support workers.Actively follows clients even when they are in the hospital.Assist in developing and implementing realistic, effective discharge planning.Regularly monitors medications of clients.Interface as needed with psychiatrists concerning medications and other related issues.Education andExperience: Bachelor's degree in social work or other closely related fieldMinimum Qualifications: Ability to pass background check and driver's licensecheck upon offer of employmentJob Type: Full-timeSalary: $0.00 /yearExperience:Relevant: 1 year (Required)Education:Bachelor's (Required)Work Location:One locationBenefits:Health insuranceDental insurancePaid time offPay Frequency:Bi weekly or Twice monthlyThis Job Is Ideal for Someone Who Is:Dependable -- more reliable than spontaneousPeople-oriented -- enjoys interacting with people and working on group projectsAdaptable/flexible -- enjoys doing work that requires frequent shifts in directionThis Company Describes Its Culture as:People-oriented -- supportive and fairness-focusedTeam-oriented -- cooperative and collaborativeSchedule:OtherCompany's website:WMMHC.ORGBenefit Conditions:Waiting period may apply
