At Welsh Mountain Health Center, the role of an LCSW or LPC is to assist our Primary Care Providers in recognizing, treating and managing mental health and psycho social concerns. LCSW/LPC's are hired as a traveling position as to ensure we can provide treatment to patients at our Medical offices in Lebanon and Lancaster County.

JOB PERFORMANCE EXPECTATIONS
Provide group and family therapy sessions as indicated.
Address social needs of patients and utilize area referral sources for needed specialty care, hospitalization, or advanced psychiatric assessments and treatments.
Receive internal referrals from WMHC’s physicians and mid-level providers.
Assess the mental and behavioral health status of referred patients.
Formulate differential diagnosis of mild to moderate mental health conditions and/or substance abuse.
Formulate short-term and long-term counseling plans to patients under care.
Consults with medical director and other providers regarding complex patients and regarding prescription medications prescribed by providers.
Identify patients with urgent mental health conditions (IE: suicidal, homicidal, psychotic, etc.) and make the appropriate referrals to available emergency or psychiatric treatment facilities.
Provide written documentation of counseling sessions, assessments, interventions and recommendations for WMHC’s providers.
Assist Medical Director in developing protocols for WMHC’s counseling services and criteria for outside referrals.
Prepare monthly productivity reports.
Prepare quality improvement reports.
Provide in-service training on relevant Behavioral Health topics for co-workers as requested.
Participate in a team management approach to patient care/treatment.
Maintain an understanding of how behavioral health priorities are administered under state DHS guidelines.
Consult with specialists and colleagues to review and participate in the care of patients receiving indirect/contracted medical services.
Maintain quality care through participation in peer review, policy development and quality improvement programs.
Provide non-scheduled urgent assessments of patients with medical clinic appointments.
SCHOOLING, EXPERIENCE, CERTIFICATION/S and/or LICENSURE/S
Bachelors or Masters Degree in Counseling with Professional Clinical Counselor License.
Bachelors or Masters Degree in Social Work with Clinical Social Worker License.
One (1) or more years, experience in an outpatient mental health counseling preferred.
Basic understating of primary care.
Knowledge of EMR.
Knowledge of community resources.
Bi/Multi lingual preferred.
