Licensed Practical Nurse-VNA Home Health - South York County
Under the supervision of a registered professional nurse, provides nursing care to patients at an advanced level of practice. Takes patient vital signs, assists with procedures/examinations, administers medications, assists patient with activities of daily living (bathing, feeding, etc.) and instructs patients and their families in appropriate care.
Duties and Responsibilities:
1 Under appropriate supervision, administers and controls narcotics and medications. 2 Provides general nursing care and personal care assistance to patients, including bathing, dressing, eating, changing bed linens and bed pans and positioning. 3 Maintains a safe, comfortable and therapeutic environment for patients/families in accordance with hospital standards. 4 Helps patients get into and out of chairs, lifts patients, raises patients in beds, and turns the patients. 5 Checks patient's vital signs. 6 Charts results of various tests. 7 Monitors medical condition of patients and reports changes to appropriate personnel. 8 Prepares patients for scheduled procedures to assist in patient assessment. 9 Assists physicians with examinations, procedures and other processes related to direct patient care. 10 Arranges for patients' ordered diagnostic and therapeutic services. 11 Provides health care instruction/education to patient and family members. 12 Ensures accurate maintenance of patients' medical records and that patient records are treated as confidential information. 13 Performs a variety of nursing administrative duties. 14 Ensures an adequate stock of supplies and proper functioning of equipment.
WellSpan VNA Home Care provides skilled nursing, physical therapy, occupational therapy, speech therapy, home health aid and social work visits to patients who require intermittent health care in their homes. Care is primarily focused on short episodes (6-8 weeks) of recovery after the surgery, the onset of a new diagnosis or the exacerbation of an existing condition, and is ordinarily covered by insurance. Employees work together to provide exceptional, quality care to members of the York and Adams County communities, via direct patient care or support services enabling the care to be provided.

Minimum Education Post High School Vocational/Specialized Training
Minimum Experience 1 - 2 years
Req Cert/Registration Current LPN license in PA; Basic Life Support (BLS) certification required
Req Course(s)/Training Completion of accredited LPN program; rhythm recognition course per unit of assignment
Prolonged, extensive, or considerable standing/walking
