CHAP/CHEER Case Manager
We have an immediate opening for a Case Manager for our Community Housing Assistance Program (CHAP)/ Community Housing Employment Enrichment Resources (CHEER). This is a grant funded, long-term, part time position for the provision of case management services for young adults. The position’s schedule is flexible for an initial fifteen hours per week with an opportunity to increase in hours as cases are added up to 30 hours per week. The hourly rate is $16.50 per hour.
The Candidate must have experience in teaching and reinforcing life skills and assistance with community link ages for development of natural supports. The ability to work professionally with others, as well as autonomously is required.You must have Bachelor’s Degree in a closely related field (Social Work, Secondary Education, Counseling, etc.) and a minimum of two years of experience working with youth. It is required to possess excellent interpersonal communication and writing skills, be well organized, the ability to manage your time effectively, and operate business computer applications at an intermediate level (Microsoft Word, Excel, Business Email Systems, etc.).
You will be required to submit 3 professional references,have a clear DCF and criminal background check, obtain a medical clearance to work, and have a clear motor vehicle report. You must have a valid CT Driver’s License and proof of auto insurance. Bilingual or multilingual (English/Spanish/other)is a plus but not mandatory.
