Work Hours: Monday to Friday 8am-3:30pm or 7:30am-3:00pm (no work on the weekends!)

WORK Inc., one of New England’s most progressive providers of services to adults with disabilities is seeking exceptional persons who are committed to work that make life better for others. Currently, we are looking for a full time Case Manager who will support individuals served in the Community program by promoting independence, while empowering and enhancing their quality of life.

 

Responsibilities include:

  • Development and maintenance of individualized service plans to address client need areas, using evaluations and assessments
  • On-going monitoring, collection, and assessment of program data and completion of monthly and quarterly progress notes.
  • Serving as a primary liaison between the Ability One Program and the community, including families, residential staff, DDS personnel, DPPC, etc.
  • Preparation and implementation of program goals, individuals’ service plans, treatment plans, etc.
  • Maintenance of client files for a caseload of individuals
  • Assisting in the transition of individuals with disabilities from work program model to supported employment &/or community based activities

Job Qualifications

  1. Bachelor’s degree in Human Services or related field, And minimum one year’s experience working directly with individuals with developmental disabilities
  2. Professional, responsible and punctual
  3. Active/Valid Massachusetts Driver’s License
  4. Good Communication Skills
  5. Have a Team Player mentality
  6. Good organizational skills
  7. Proactive mentality
  8. Some knowledge of the HCSIS reporting system is a plus (or willing to learn)

Preference will be given to candidates who can work the scheduled hours of 8am-3:30pm or 7:30am-3:00pm, M-F. It is essential that the candidate can be prompt to their scheduled time to work.
