Job Description

The WEDCO District Health Department & Home Health Agency service array includes preventive health care such as Well Child and Prenatal Clinics; early intervention and treatment clinics such as STD and TB; environmental services such as restaurant, swimming pool and hotel inspections; health education services such as drug prevention and postponing sexual involvement education; as well as many other vital services such as home visitation to high risk parents (our HANDS Program) and school nursing.

This position is a social worker position which performs visits to parents and their children in the home setting for WEDCO's HANDS program (Health Access Nutrition Development). Applicants must be willing to provide planned home visits for voluntary home visitation program to assist parent(s) and/or families with child development, parenting skills, health services and linkage to other needed resources. Candidate will be responsible for the scheduling of weekly visits and meeting with Supervisor as outlined in the HANDS program guidelines.

Minimum Education, Training or Experience: Bachelor Degree with a major in Social Work.

Special Program Requirement/Substitution for Education, Training or Experience: Major in Sociology or Psychology may be considered for this position if the individual has at least one (1) year of experience in case assessment and management.

Position will be based in Scott County; however, travel will be required to Nicholas and Harrison Counties as needed. Travel expenses are reimbursed.

Grade 14 - Salary range is $11.40-$14.18 per hour.

General Duties include: This position serves under moderate direction of the Director or other appropriate coordinator/supervisor assigned.

  1. The responsibilities include home visitations that may take place in the client’s home or another community site if justified.
  2. Visits include monitoring of the child’s, mother and family’s progress by making referrals to community resources.
  3. Tracking appointment to ensure they are being kept.
  4. Perform follow up services as identified.
  5. Perform periodic evaluations of the participants changing needs.
  6. Preparation and maintenance of case records which shall be documented with contacts, services needed, reports, and progress.
  7. Consultations
  8. Crisis assistance.
  9. Provide regular planned visits to establish a trusting relationship for voluntary eligible HANDS participant(s) to assist first time parent(s)/, Multigravida parent(s) and or families with child development, parenting skills, health services and other needed resources identified at assessment or during visits.
  10. Will complete Parent Surveys and RN/SW Visits.
  11. Works in a mixed- role as a Parent Visitor, Family Support Worker and RN/SW Visitor.
  12. Provides FSW Sub Pro visits as needed.
  13. Conduct initial home visit and complete assessment with first time parent(s)/Multigravida parent(s)  and or families, eligible, voluntary participant, to establish rapport and planning for providing services as qualified by the provider.
  14. Will function in role of Parent Visitor as needed, providing referral packets to families participating in the Parent survey Process and offering all families information and referral to community resources and intensive home visitation services based on availability within caseloads.
  15. Document case record and record home visit logs accurately on each contact, progress, service need, and complete reports as required.
  16. Facilitate families in securing of prenatal/child health services, referrals, and appointments with other supportive agencies such as resource centers, health care providers, schools, Head Start/Early Head Start and Family Preservation services.
  17. Tracking appointments to ensure they are being kept, performing follow-up services as identified and performing periodic evaluations of the participants’ changing needs.
  18. Confer with Program Coordinator and/or Supervisor in cases involving potential referrals and/or collaboration with other service providers.
  19. Facilitate families in securing of prenatal/child health services, referrals, and appointments with other supportive agencies such as resource centers, health care providers, schools, Head Start/Early Head Start and Family Preservation services.
  20. Tracking appointments to ensure they are being kept, performing follow-up services as identified and performing periodic evaluations of the participants’ changing needs.
  21. Confer with Program Coordinator and/or Supervisor in cases involving potential referrals and/or collaboration with other service providers.
  22. Maintain weighted caseload of 18-22 in FSW role, provides RN/SW visits and Parent Surveys. (Goal: 3-4 completed home visits per day with an average of 60 visits per month)
