Emergency Residential Services - Safe Housing for Veterans
The Department of Veterans Affairs' (VA) Plan to End Homelessness Among Veterans calls for enhancing current homeless service capacity as well as developing new programs and initiatives in concert with community and federal partners. The intent of this program is for VOALA to offer housing and supportive services to twenty-five (25) homeless Veterans in a safe and secure environment that supports their goals for recovery from homelessness. VOALA will be able to provide safe housing for up to ten (10) women at any given time based on the need of the community. VOALA services will be offered within the catchment areas of the VAGLAHS. VOALA is required to provide a therapeutic and rehabilitative milieu and attendance services targeting the underlying factors contributing to homelessness. VOALA is not required to provide detoxification or other hospital level treatment.
The goal of this program is to rapidly stabilize Veterans' medical, mental health, substance abuse and other psychosocial problems in order to place Veterans in other appropriate transitional or permanent housing within 90 days.
JOB SUMMARY AND PURPOSE
The Case Manager - Specialist is responsible for helping people who are in difficult situations with advice, figure out what kind of help they need, help them find the services they need, create plans for treatment or recovery, work with other health and human service providers, and keep tabs on client's progress with treatment plan. The position requires a combination of administrative/office duties and field/outreach work.
DUTIES AND RESPONSIBILITIES
The Case Manager - Specialist will arrange, coordinate, monitor, and deliver services related to meeting the needs of program households, while helping them obtain housing stability. The Case Manager - Specialist duties are as follows:
Assign and monitor cases:
Review daily referrals and screenings of clients
Designates households for intake and ensures all supporting documents are verified
Provides information and referrals for callers and walk-in clients
Conducts comprehensive screenings and assessments with clients to collect functional, environmental, financial, employment, housing, educational, and health information, as appropriate, to develop a case plan
Develops support systems to meet clients and family needs; coordinates and facilitates move-in activities, rental negotiations, and service referrals as required
May make home and field visits
Monitors cases on a regular basis to determine quality and effectiveness of services provided.
Review cases with Program Coordinator and Case Management Team.
Audit case file on a regular basis for completeness and accuracy
Conducts crisis intervention as necessary
Assists case managers by identifying and coordinating a variety of available services necessary for clients and families to maintain/obtain self-sufficiency and family stabilization
Refers clients to available community providers for therapeutic, social, educational, and medical resources
Provides support to case managers and clients through ongoing case conferencing, counseling, and advocacy
Conducts comprehensive file reviews and assessment of data collected to assure all necessary documentation is collected by case managers for services eligibility purposes
Assure all assets and file documents are maintained and secure for all clients
Develops, performs, and assists with workshops pertaining to client's needs.
Participates and assists with outreach events
Provide Crisis intervention and stabilization
Meets with participants on a weekly basis to establish goals and action steps towards those goals, and to monitor and assess progress towards achieving those goals
Collect demographic and evaluative data on participants and their progress weekly
Actively participate in care coordination teams bi-weekly to ensure an intensive wrap-around approach to service delivery, including aftercare for youth
Other duties as required and is subject to change at any time
Must be able to pass a background check
Valid California driver's license, acceptable proof of automobile insurance, and acceptable driving record to be covered by VOA's insurance carrier
Bachelor's Degree in related field from a full-accredited college or university
Minimum of 2 years of experience as a case manager
Minimum of 2 years of experience providing Intensive Case Management trauma-informed care to vulnerable youth and/or relevant experience working with veterans, people in recovery, or homeless individuals
Case Management ethics, HIPAA, Programmatic standards of operation and clinical engagement, clinical documentation methods, client focused ideology.
Proficiency in record keeping, tracking, maintaining, and reporting statistics according to program guidelines and requirements
Proficiency with computers and Microsoft operating system navigation, office, outlook, word, excel, power-point, and internet browser
SKILLS AND ABILITIES:
Ability to develop strong working relationships with homeless youth, their families, and landlords, as well as a variety of service stakeholders (schools, health and public benefit agencies.
Ability to perform under circumstances of possible emotional stress and conflict, including dealing with un-cooperative clients.
Effective written and oral communications skills.
Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA, and other federal, state, and local standards, with or without reasonable accommodation, including meeting qualitative and quantitative productivity standards.
Ability to maintain regular, punctual attendance consistent with ADA, FMLA, and other federal, state, and local standards
Dependable, prompt, client-focus, collaborative, prompt, positive mindset, flexible, and organized
3 years of experience working with U.S Armed Forces Veterans, homelessness, re-entry population, human trafficking, substance abuse patients, and/or at-risk youth
Experience in working in a Group home and Transitional living setting
Bilingual in English and Spanish (written and verbal)
Knowledge and experience working with and using HMIS software or programs
Knowledge and experience working with and using Database software programs
