Job Description

 

Our new free-standing Memory Care Community is opening soon and will be staffed as a true Household Model with the focus on self-led universal workers who form deep relationships with the residents they serve. The three-story community will feature three 16-residence households (two assisted living and one skilled nursing), each with its own shared community living and its own therapy space, salon and chapel.

We are currently seeking full-time Universal Household LPN. Scheduled Hours - Flexible schedules. A Multistate or Kansas license is required. The shift is a rotating 7a-7p with every other weekend.   Also have an opening for rotating  7p-7a and every other weekend.

Our universal workers perform a portion of their schedule in a primary functional role with the remaining portion in a universal role performing dining, housekeeping and activity services to meet our residents’ needs.

PRIMARY/ESSENTIAL DUTIES
The Household LPN must be capable of performing the following primary and essential duties along with other duties as necessary:
• Clinical Service Delivery: Plans, oversees and reviews services delivery and completion of nursing duties by CMAs and CNAs. Assists team members with resident services and activities of daily living as necessary.
• Communication and Reporting: Receives report from off-going shift and gives report to oncoming shift. Communicates about resident condition with other team members; communicates changes in resident condition.
• Care Planning: Within licensure, performs resident assessments upon admission; completes documentation for assessments, evaluates resident response to plan of care and recommends additional modifications as indicated.
• Medication and Treatments: Administers medications and treatments as prescribed by physician and nursing orders, according to regulations and established procedures.
• Clinical Rounds: Performs clinical rounds. Educates residents (and others) about medical issues and healthy lifestyle choices.
• Electronic Health Records: Maintains resident clinical records.
• Compliance: Ensures the clinical operations in the household are in compliance.
• Continuing Education: Attends in-service education programs, team conferences and nursing services conferences and completes in-service programs as assigned.
• Coaching Others: Trains other clinical team members in nursing skills, and all team members in best practices.
