Job Description

Valir PACE is a Program of All Inclusive Care for the Elderly (PACE), and is the future of healthcare. It utilizes a team-based care approach providing a personalized, comprehensive healthcare plan to the most at risk, those without access to quality healthcare, the under and uninsured. Individuals enrolled in our program have better health outcomes, less visits to the emergency room, while continuing to live in their own home, often times without ever having to go into a nursing home. The focus of every PACE organization is to help the elderly live in the community for as long as possible. To meet this goal, PACE organizations focus on preventive care.

We are currently seeking a full time RN Care Coordinator for Valir PACE.

 

The salary range for this position is $31.00 to $36.00 per hour depending upon applicable experience, PLUS a monthly cell phone reimbursement of $75.00. Full Benefits including health, dental, vision, life, disability, and retirement benefits. Responsibilities

Valir PACE provides integrated medical and social services to our most vulnerable seniors, helping to unlock their full potential. POSITION PURPOSE:

The PACE RN Care Coordinator coordinates and delivers medical care to Valir PACE Foundation participants. The RN works jointly with physicians, physician extenders, other nurses, therapy, activities, dietary, social services and transportation as part of the Interdisciplinary Team to determine and monitor the care plan for each PACE participant in compliance with all national and state regulations/laws.

Qualifications

POSITION REQUIREMENTS:

License or Certification: Active RN license without restrictions, CPR Certification, ACLS preferred

 

Total Education, Vocational Training, and Experience: At least 1 year experience working with a frail, elder population.

 

Skills and Abilities:

A strong knowledge of the Federal and State laws and regulations governing health care;

Detailed understanding of government requirements/expectations for health care compliance programs;

Strong oral and written communication, planning, and problem solving skills;

Maintains a high degree of credibility, independence, integrity, confidentiality, and trust;

Able to operate successfully in a constantly changing, fast-paced environment. Demonstrates initiative, self-motivation, practical learning skills, enthusiasm, and an ability to complete multiple tasks in a timely and accurate manner;

Competent and efficient on EMR and other computer programs;

Knows and adheres to the values and goals of the Valir PACE Program;

Maintains a safe environment for participants, visitors and staff;

Keeps confidentiality of client records, report, and discussion;

Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload in order to fulfill responsibilities;

Knowledgeable of scope of practice for licensure and functions within scope;

Is professional in appearance and manner.
