Responsibilities







Using the cognitive skills of application, analysis, synthesis, and evaluation, the Home Health Registered Nurse is responsible for using the concepts of case management to coordinate and supervise the nursing care of all patients: infant, child, adolescent, adult, and geriatric. The nursing process, (assessing, planning, implementing and evaluating care) will be the framework in which care is provided. Key functions include providing complex direct patient care, patient/family teaching, patient/family support and advocacy, and case management. The Home Health Registered Nurse provides professional nursing services in accordance with applicable state Licensure status and in compliance with University Hospital Home Health agency policies and procedures. Accountability will be monitored through direct observations; achievement of service goals including satisfaction, quality, and financial outcomes; and performance improvement activities. Works in accordance with the Physician's orders and under the supervision of the Clinical Nurse Supervisor.







Qualifications







EducationGraduate of an approved school of nursing. Must successfully complete Home Health Staff Nurse orientation/cross-training. RN II requires BSN, Baccalaureate in nursing or MSN.Licensure/CertificationCurrent and valid professional nurse license in Georgia and/or South Carolina as appropriate. CurrentAmerican Heart Association BCLS certification or higher (ACLS) required.Valid driver's license and current automobile liability insurance required. Report of Motor Vehicle Record for past 5 years required.ExperienceNormal experience gained in school of nursing required. At least one year RN experience strongly preferred. (Home Health LPN experience and/or BSN with community health training will also be considered.) Previous home health care experience is highly preferred. Additional hospital or other professional nursing experience is also a plus.Knowledge, Skills and AbilitiesAbility to be flexible.Ability to think critically and prioritize multiple tasks and responsibilities.Nursing knowledge of growth and development relevant to the ages of the patients served.Nursing knowledge of the diseases and health care needs of the patient population served.Assessment, analytical, and synthesis skills sufficient to interpret and analyze patient health data; identify nursing diagnoses; prioritize needs; and develop an individualized plan of care.Ability to perform nursing functions.Evaluation skills adequate to evaluate patient response to care and take appropriate action. High level of interpersonal skills in order to effectively communicate with patients, families, physicians and other home health care personnel with whom daily interactions must take place.Basic knowledge of and skills in management sufficient to effectively supervise other home health staff.Problem solving skills sufficient to allow effective problem/ conflict resolution.Ability to perform job assignment with minimal supervision.Ability to provide reliable means of transportation.Demonstrated traits of initiative and dependability required.Ability to perform efficiently and effectively under stress.Philosophy of health care service is compatible with that of the mission of University Hospital.Physical DemandsStanding for long periods of time (6-8 hrs.).Sitting for extended periods of time (at desk telephoning, making schedules, writing reports).Full range of motion including handling, lifting and turning patients, and transferring patients from chair to bed.Bending and stooping (as needed to provide nursing services, treatments and preventative procedures as required).Frequent walking.Lifting of heavy patients and supplies (supplies weighing approximately 25 pounds).Sensory: sight to observe patient symptoms auditory to hear patients tactile to feel patient symptoms (warmth, induration) olfactory to smell disease specific smells
