Job Description

INTRODUCTION

Under the supervision of the Infectious Disease (ID) Nurse Coordinator, the Clinical Nurse, Infectious Disease provides clinical, educational, and administrative services to clients seen by Infectious Disease Specialists. This position works with the providers, clients, Unity staff members, and external referrals to ensure care is available according to Unity’s standard of care.

MAJOR DUTIES/ESSENTIAL FUNCTIONS

Essential and other important responsibilities and duties may include, but are not limited to the following:

  • Provides safe, therapeutic care to clients in a holistic and systematic way
  • Utilizes standards, guidelines and protocols for care delivery and incorporates data to continuously improve care and outcomes.
  • Provides case management to complex patients with HIV infection, with focus on clients who are pregnant, newly diagnosed, have a CD4 less than 200, a detectable Viral Load, and/or at risk.
  • Educates and monitors clients who are undergoing Hepatitis treatment, assists in prior authorization process
  • Coordinates care with ID team physicians. Within the CHC, this may include travel with the physician or self to ensure care at various sites within the Unity Health Care network
  • Completes nurse assessment documentation for patient encounters, and completes an acuity scale when appropriate.
  • Expedites prior authorization process to ensure access to medications, and avoid treatment interruption
  • Checks medications prescribed, and verifies that patient understands treatment plan. Reviews potential side effects of medications and medication list with client
  • Assists the patient in developing a medication schedule and individualized adherence strategies as needed
  • Assesses barriers to care, client concerns, and makes the appropriate referrals and follows up.
  • Works closely with team to track clients who are disengaged from care and assists in re-engagement
  • Collaborates with primary care, other specialists and outside agencies to ensure communication amongst provider
  • Participates in trainings works with supervisor to support personal and professional growth
  • Performs other duties as assigned.

MINIMUM QUALIFICATIONS

  • Associated degree in Nursing required. Bachelors of Science in Nursing is preferred.
  • Current DC RN license.
  • Two (2) years of case management experience or two (2) years of clinical nursing experience in an acute care, correctional health and/or ambulatory care setting
  • One (1) year working with HIV/AIDS patients is preferred.


REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES

  • Ability to assist in directing the care of the HIV/AIDS patients.
  • Excellent oral and written communication skills.
  • Ability to articulate Unity’s mission through his/her work.
  • Demonstrated ability to effectively work and communicate with diverse populations.
  • Demonstrated proficiency with business software (e.g. Microsoft Office Suite).

SUPERVISORY CONTROLS

The position reports to the Infectious Disease Nurse Coordinator. This position also reports to the DOC Health Care Director for day to day operations while in the DOC.

GUIDELINES

The position abides by all rules and regulations set forth by applicable licensing and regulatory bodies, as well as UHC policies and procedures.

PERSONAL CONTACTS

The position requires contact with staff at all levels throughout the organization. There are also external organization relationships that may be a part of the work of the incumbent. The incumbent will be expected to maintain a professional relationship with all HIV clients and referral sources.

PHYSICAL EFFORT AND WORK ENVIRONMENT

  • Must be physically able to sit, stand, and walk for long periods of time. Must be able to bend, lift, and carry files from one location to another.
  • Must have visual acuity and the ability to differentiate colors, and sustain long periods of computer usage.
  • May sit for prolonged periods of time at a desk or in an automobile, and/or may use the telephone for long periods of time.
  • The office environment may be stressful with multiple, time-sensitive tasks to be accomplished within a short period of time.
  • Must be able to work any time of the day, independently with minimal supervision, be capable of making sound business and decisions, and be detail oriented, alert and self-motivated.
  • Must be able to effectively manage difficult situations, staff, and customers.

OTHER SIGNIFICANT FACTORS

The incumbent must be able to balance the needs of diverse constituencies on a daily basis.

RISKS

The position’s work involves everyday risk and discomforts, which require normal safety precautions typical of such places as offices, meetings, training rooms, and other UHC sites. The work area is adequately lit, heated, and ventilated. The employee shall provide evidence of a recent (within the past twelve (12) months) health assessment that includes a PPD and/or chest x-ray results.
