We are looking for a full-time CMA/LPN to join our growing team. This position will be a key part of our structured, collaborative model that is transforming the delivery of care in the post-acute care setting. Our staff is directly involved in the improvement of clinical outcomes and the patient experience. With a high-touch approach, TCPA helps ensure proper progress through a comprehensive plan of care which will help identify any early warning signs for readmissions during the critical, immediate post-discharge period from the facility. This CMA/LPN position will work closely with the geriatric population, and patients in a rehabilitative environment. Daily travel to our client facilities(Skilled Nursing, Assisted Living, etc)
- Obtain Vitals, and document in EMR
- Assist in chart preparation for Physicians/Nurse Practitioners
- Phlebotomy for lab orders (certification preferred)
MUST HAVE CURRENT GEORGIA LPN or CMA license to APPLY!
