- Deliver in-home, in-school, in-community or in-office services to clients as requested by the area mental health including the specific hours requested. These services may be provided in the school setting by therapists identified as members of the school based therapy unit if the client’s presenting issues would be better addressed in that setting.
- Provide therapeutic interventions to the identified client in order to enhance and foster the client’s optimal functioning and interactions in the home and community.
- Provides individual Psychosocial Skills Rehabilitation (PSR) services in the community.
- Assists clients in restoring, rehabilitating, and developing social and interpersonal skills.
- Identify, enhance, and establish support networks, community awareness, coping strategies, and improve functioning in the individual’s social environments (home, work, school).
- Help restore, rehabilitate, and support the development of daily living skills.
- Help improve each client’s self-management of the negative effects of psychiatric or emotional symptoms.
- Assists clients in identifying, effectively responding to or avoiding identified precursors (“triggers”) that lead to functional impairments.
- Help implement Crisis Management Plan on an ongoing basis.
- Assists clients in developing goals for treatment within the first 30 days of service.
- Continually assess client needs, service availability and appropriateness.
- Communicate client’s status on a regular basis, including, but not limited to changes in mood or functioning, medical issues and all crisis and health and safety issues.
- Provide Crisis Intervention services to recipients as needed according to agency policies, procedures, and service definitions.
- Work cooperatively with treatment team members and the family in the provision of services to meet the goals developed in the Person Centered Plan.
- Actively participates in all evaluations conducted by the facility.
- Provide the appropriate documentation for service delivery including treatment goals, progress notes and billing and submit said documentation as specified by Medicaid standards.
- Maintain caseload and well organized schedule to meet productivity expectations
- Attend weekly staff meetings for agency communication, clinical consultation and training
- Participate in collaborative meetings with direct supervisor, other agencies or community and natural supports as needed.
- Be knowledgeable of and adept at producing effective, clinical documentation of every contact with recipient.
- Turn in notes on time according to policy. Document all “no shows,” and communicate all difficulties contacting clients with appropriate office staff.
- Be able, or willing to learn, to use computer interface to complete documentation for billing or other purposes as needed.
- Attend all office trainings as well as regular clinical supervision sessions with Clinical Manager and/or Team Manager.
- Document services provided regularly, concisely, and completely in accordance with clinical standards by office deadline.
- Return phone calls promptly
- Follow all safety rules
Requirements:
- Minimum 1-2 years behavioral/mental health/special education experience
- Bachelors Degree in one of the following: counseling, social work, psychology, or sociology
